Amendment 0004 of the SOURCES SOUGHT NOTICE HAS BEEN RELEASED TODAY AND IS POSTED ON THE PROCUREMENT GATEWAY'S WEBSITE AT: http://progate.daps.dla.mil/home/ DEFENSE SUPPLY CENTER PHILADELPHIA (DSCP) DIRECTORATE OF MEDICAL MATERIEL SOURCES SOUGHT NOTICE for SURGICAL GRADE INSTRUMENTS Amendment 0004 Issue Date: March 10, 2004 Reference the Request for Samples/Literature to locate potential sources for the standardization of surgical grade instruments, within the Department of Defense (DoD) and the Department of Veterans Affairs (VA), to secure the best possible products and prices for their respective hospitals and clinics, worldwide issued February 9, 2004, Amendment 0001 issued February 18, 2004, Amendment 0002 issued March 4, 2004, and Amendment 0003 issued March 8, 2004. REVISED - Submission Deadline: The closing date remains Wednesday, March 17, 2004. All submissions must be received by 3PM, Central Standard Time (CST), on Wednesday, March 17, 2004. REVISED - General Information: 1. On Page 2 of the Sources Sought Notice for Surgical Grade Instruments, under General Information, DELETE paragraph 2 in its entirety and SUBSTITUTE “2. For a submitter to be eligible for the BPA, each submitter must have at least 55 of the 74-item types pass all stages of the evaluation. Alternate proposals will be evaluated independently of each other. For example, a failure under the initial proposal doesn’t mean a failure under the alternate proposal.” 2. On Page 2 of the Sources Sought Notice for Surgical Grade Instruments, paragraph 3.b. – Representative sample failure during any other portion of the evaluation – remains as written. 3. On Page 2 of Amendment 0001, Paragraph B is DELETED in its entirety. REVISED - Technical Evaluation Factor 4 – Detailed Product Literature Evaluation: The requirement for the Consolidated catalogs to have a “Table of contents” has been DELETED. REVISED – Surgical Instrument Groups – Product Specific Information: Reference Page 18, Abdominal Self-Retaining (BALFOUR) Retractors. Under paragraph D (Abdominal Self-Retaining Retractor Visual Inspection and Actual use) ADD the following: “5. Provides user comfort that enhances surgical technique, as determined by the SI-CPT member.”
