- (i) This is a combined synopsis / solicitation for commercial items prepared in accordance with the format in FAR 12.603, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are hereby requested because a written solicitation will not be issued. (ii) Solicitation No. FA4621-04-Q-0024 is being issued as a Request For Quotes (RFQ). (iii) This solicitation incorporates provisions and clauses in effect through the Federal Acquisition Circular 2001-20, dated 23 Feb 2004. (iv) This solicitation is synopsized as a 100% small business set-aside under NAICS Code 339114, 500 employees. (v) Please quote to supply 0001 Power Control Console 2i without irrigation, OsteoMed Corp P/N 450-0021, or equal (QTY 1); 0002 Modular Motor Unit and Cord with foot switch control only, OsteoMed Corp P/N 450-0084, or equal (QTY 5); 0003 Series 3 1:1 Straight Drill, OsteoMed Corp P/N 450-0777, or equal (QTY 5); 0004 Bi Directional Master Foot Switch, OsteoMed Corp P/N 450-0390, or equal (QTY 1). (vi) “Requested Delivery:” 1 Apr 2004; Place of delivery: Building 250 at McConnell AFB, KS 67221; FOB Destination. (vii) Provision 52-211-6, Brand-Name or Equal applies to this acquistion.
