Description

Contractor shall deliver, transport, set-up, and takedown of shower trailers at Gila Bend AFB in accordance with Statement of Work. Shower Trailers shall be in place at Gila Bend AFB, in Arizona on 19 March 2004 and shall be removed on 28 April 2004. All interested in submitting proposals please contact SSgt Elizabeth G. STATEMENT OF WORK PORTABLE SHOWERS GILA BEND AFB, AZ 25 FEB 2004 SETUP AND TAKEDOWN LOCATION: GILA BEND AFB, AZ GENERAL: This project involves the delivery, transportation, set-up, takedown, and pick-up of portable shower units at Gila Bend AFB, Arizona. PROJECT DESCRIPTION: The contractor shall provide all materials, transportation, equipment, labor, parts, training, and cleanup necessary to perform to following tasks 1. Provide portable shower trailers consisting of 10-12 shower units each. Contractor shall provide all connections from existing electrical and water sources to the trailers. Contractor shall be available to repair/replace any inoperable equipment from 19th March 2004 ? 28 April 2004. 2. Contractor shall provide all necessary generators needed to provide lighting inside the trailers. ITEMS AND DATA TO BE FURNISHED BY THE GOVERNMENT: The AF POC will coordinate site access and site layout to include any equipment storage requirements. DEVIATION FROM SCOPE OF WORK: The contractor shall not depart from nor perform work outside of this Statement of Work and criteria on which the Statement of Work is based without the written direction of the Contracting Officer (CO). If the contractor performs such work without written approval, it does so at its own risk. Any changes to the SOW will be coordinated with the Contracting Officer and the 18th Army Command. ACCEPTANCE OF WORK: The AF POC and the Capt Glass will approve and accept all work. SITE VISIT TO BASE: A site visit shall be conducted on 8 March 2004, at 1:00 pm, all private sources and government agencies (federal, state, county and city) shall contact SSgt Elizabeth Hanson from the 56th Contracting Squadron, 623-856-7179 ext. 235 for the number of individuals you have participating. The site visit shall be at Gila Bend, AZ Auxiliary Field. OTHER REQUIREMENTS: SUBCONTRACTORS: The contractor shall not enter into any subcontracts that are not identified in its original proposal without prior written approval of the CO. The Contractor agrees to comply with the provisions of this paragraph (a) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records-Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to the right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times, the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (b) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of the basic clause, the Contractor is not required to flow down any FAR clause, other than those in paragraphs (i) through (vi) of this paragraph in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause-- (i) 52.219-8, Utilization of Small Business Concerns (October 2000) (15 U.S.C. 637(d)(2) and (3)), in all subcontracts that offer further subcontracting opportunities. If the subcontract (except subcontracts to small business concerns) exceeds $500,000 ($1,000,000 for construction of any public facility), the subcontractor must include 52.219-8 in lower tier subcontracts that offer subcontracting opportunities. L-722 Site Visit A site visit is scheduled for all interested parties on 8 March 2004 at 1:00 pm. Bidders/offerors should be at the Gila Bend AFAB meet at the corner of 4th street and A Street to view the work site and present pertinent questions. Bidders/offerors should visit the site and take such other steps, as may be reasonable necessary to ascertain the nature and location of work and the general and local conditions that can affect the work or cost thereof. Failure to do so will not relieve bidders/offerors from the responsibility for estimating properly the difficulty or cost of successfully performing the work. The government will assume no responsibility for any understanding or representation concerning conditions made by any of its officers or agents prior to the execution of the contract, unless included in the invitation for bids/request for proposals, the specification, or related documents.
