Amendment 0002 to THE SOURCES SOUGHT NOTICE HAS BEEN RELEASED TODAY AND IS POSTED ON THE PROCUREMENT GATEWAY'S HOME PAGE AT: http://progate.daps.dla.mil/home/ DEFENSE SUPPLY CENTER PHILADELPHIA (DSCP) DIRECTORATE OF MEDICAL MATERIEL SOURCES SOUGHT NOTICE for SURGICAL GRADE INSTRUMENTS Amendment 0002 Issue Date: March 4, 2004 Reference the Request for Samples/Literature to locate potential sources for the standardization of surgical grade instruments, within the Department of Defense (DoD) and the Department of Veterans Affairs (VA), to secure the best possible products and prices for their respective hospitals and clinics, worldwide issued February 9, 2004 and Amendment 0001 issued February 18, 2004. REVISED ? Price Quotations: On page 1 of the Sources Sought Notice for Surgical Grade Instruments, under Price Quotations, ADD the following to the end of the paragraph: ?At time of price quotation, the submitter will be given the opportunity to offer prices on the additional sizes for the specified item types from the same manufacturer (as shown in the Surgical Instrument Groups ? Product Specific Information) presuming each will be covered by the FSS at time of issuance of the BPA (see Paragraph 2 under Initial Screening of each Submission for additional information). Submitters offering additional sizes and pricing as part of their price quotation may be considered to be overall better value to the Government than one that doesn?t. REVISED - Submission Deadline: Reminder: Submission must be received by 3PM, Central Standard Time (CST), on Wednesday, March 10, 2004. REVISED ? Initial Screening of each Submission: A. On Page 3 of the Sources Sought Notice for Surgical Grade Instruments, under Initial Screening of each Submission, DELETE the following ?An initial Pass/Fail screening will be conducted of all submissions to ensure they meet the below listed requirements. Submissions that do not pass all elements of the initial screening will not be eligible to receive the BPA. The screening will verify the following:? and SUBSTITUTE the following: ?An initial Pass/Fail screening will be conducted of all submissions to ensure they meet the below listed requirements. The burden is on the submitter to make sure its submission meets all Initial Screening requirements set forth below by March 10, 2004. However, during the initial Pass/Fail screening, the Government will notify the submitter if it identifies any failure to meet these requirements, so that the submitter may make its submission compliant with the requirements. If the submission is not compliant within three business days of notification, the submitter will be ineligible to receive the BPA. B. On Page 3 of the Sources Sought Notice for Surgical Grade Instruments, under Initial Screening of each Submission, under Paragraph 2, ADD the following: Submitters intending to offer additional sizes from the same manufacturer of the specified instrument types MUST submit the following typed on submitter letterhead: a. List each additional size to be offered on the price quotation; and b. Certify that the additional sizes are of the same quality as the samples submitted for evaluation. C. On Page 2 of Amendment 0001 to the Sources Sought Notice for Surgical Grade Instruments, under the REVISED - Initial Screening of each Submission, DELETE Paragraph B, and SUBSTITUTE the following new paragraph B: ?B. On Page 3, under Initial Screening of each Submission, under Paragraph 3, DELETE the first sentence and SUBSTITUTE ?For all instruments, the ?working end? of the instrument sample must have the required dull, unpolished finish (also known as satin finish).? D. On Page 3 of the Sources Sought Notice for Surgical Grade Instruments, under paragraph 7 ? Product Literature Review, ADD the following to the end of the first sentence: ?as well as for each additional size it will offer under the price quotation.? REVISED - ELIGIBILITY REQUIREMENT FOR BPA: On Page 4 of the Sources Sought Notice for Surgical Grade Instruments, under Paragraph 1, DELETE the first sentence and SUBSTITUTE ?Submitters must have a VA Federal Supply Schedule (FSS) contract for all surgical instruments it intends to provide through the BPA, prior to issuance of the BPA.?
