Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement is a 100% set-aside for small business only. Solicitation number Q693-1-04 is issued as a Request for Quote (RFQ). This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2001-19 effective 2 Jan 2004. The Government intends to award five contract line item numbers for cleaning and custodial services at the Allentown VA Outpatient Clinic, 3110 Hamilton Boulevard, Allentown, PA 18103: CLIN (Contract Line Item Number) 0001, Base Year (22 March 04 to 28 March 21); CLIN 0002, First Option Period (22 March 05 to 21 March 06); CLIN 0003, Second Option Period (22 March 06 to 21 March 07); CLIN 0004, Third Option Period (22 March 07 to 21 March 08); and CLIN 0005, Fourth Option Period (22 March 08 to 21 March 09). This combined synopsis/solicitation is prepared in accordance with the Simplified Acquisition Procedures in FAR 13 and the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the O NLY solicitation; quotes are being requested and a written solicitation WILL NOT be issued. The statement of work (SOW): ?AOPC Housekeeping SOW Q693-1-04? is incorporated into this RFQ and can be downloaded from this web site. AN ON-SITE TOUR OF THE ALLENTOWN CLINIC WILL BE MADE AVAILABLE TO INTERESTED PARTIES ON 9 MARCH 04 AT 10 A.M. AND 1:30 P.M. LOCAL TIME. If interested, contact the Contracting Officer by email to indicate your time choice for the on-site tour. This solicitation is issued as a RFQ under Q693-1-04. This RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-19. This action is set aside for small business; the NAICS code and applicable size standard is 561720 and $14 Mil. Evaluation criteria: The RFQ includes the Commercial Items evaluation provision (52.212-2) which provides that award will be made to the responsible contractor whose quote is most advantageous to the Government, price and other factors considered. The significant evaluation factors are i) price and ii) past performance. The provision expresses that past performance is equal to price. The following provisions and clauses apply to this acquisition. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. The following VAAR clauses and provisions are added by addendum: 852.233-70, Protest Content; 852.233-71, Alternate Protest Procedure; 852.237-70, Contractor Responsibilities; 852.270-4, Commercial Advertising; 852.270-1, Representatives of Contracting Officers. PARTIES INTERESTED IN RESPONDING TO RFQ may submit their quote in accordance with standard commercial practice (i.e. quote form, letterhead, etc.) and MUST INCLUDE THE FOLLOWING INFORMATION: Solicitation number; offeror?s complete mailing and remittance addresses; discount terms; unit prices and extended total price for CLINS 001 through 005; DUNS number; and COMPLETED FAR 52.212-3 Alt I and three references for evaluation of past performance of same or similar services. Quotes must be received at Wilkes-Barre VA Medical Center, 1111 East End Boulevard, Wilkes-Barre, PA 18711 no later than 15 March 04 at 2:00 p.m. local time. Potential offeror?s need to be registered in the CCR.
