Description

Provide A/E Services for Project #692-04-107, entitled “Radiology Site Prep” at the Department of Veterans Affairs, VA Southern Oregon Rehabilitation Center & Clinics, White City, Oregon. Project involves the preparation of the Radiology Suite Building 201LW for the replacement of the Rad/Tom x-ray equipment. Reception area will be enlarged. A toilet and a dressing room will also be added. Work includes the renovation of the space which will require carpentry, electrical and replacement of a transformer and associated mechanical and plumbing, etc. Services will include Phase I involving the preliminary investigative services and concept design; Phase II involving the preparation of construction contract drawings and specifications for construction bidding purposes, with electronic format submission after completion of Phase II; and Phase III will be for construction period services during actual construction work. Evaluation factors to be considered will be (1) Specialized experience of the firm with similar contracts; (2) Past record of performance on similar contracts; (3) Geographical location and facilities of the working office; (4) Specific experience and qualifications of personnel proposed for assignment to this project. Estimated construction cost range is between $100,000 to $250,000. NAICS: 541310/Size Standard $4.0 million. This is a small business set aside for firms located within the State of Oregon. Firms interested and desiring consideration shall submit Standard Forms 254 & 255 by close of business, March 29, 2004 to the Contracting Officer shown above. Award is pending availability of funds.
