- The Operational Contracting Division, Commercial Acquisition Branch at Hanscom AFB, MA is issuing a solicitation FA2835-04-T-0005. The duration of this Simplified Acquisition will be done for the Period of Performance of one (1) Basic Year for 01 May 04 - 30 April 05 and four (4) Option Year Periods, Option Period 1 from 01 May 05 - 30 April 06; Option Period 2 form 01 May 06 - 30 April 07; Option Period 3 from 01 May 06 - 30 April 07 and Option Period 4 from 01 May 07 - April 08. This procurement is a Small Business Set-Aside. The NAICS Code is 621399 and the Small Business Standard is $6,000,000. The contractor shall furnish Clinical Dietician Services for Government beneficiaries at the 66th Medical Group and Health and Wellness Center at Hanscom AFB, MA 01731. These duties will include all labor, education and training, nutritional evaluations and recommendations, consultations and reports. The provider shall work within Clinic duty hours, not to exceed 1248 labor hours per Fiscal Year. Health Insurance is not provided. Performance shall be according to the requirements provided in the Statement of Work. All respondents must provide pricing for Base Year and all Option Years to be considered. Proposals that do not include prices for all Option Years will not be considered. All respondents must complete FAR 52.212-3, Offeror Representations and Certifications-Commerical Items to be submitted with proposals. This may be accessed at http://farsite.hill.af.mil. Forest, Contracting Officer, 781-377-2113. Award will be made to the offeror whose offer is the best value to the Government considering price and past performance. Offerors are required to submit at least three (3) references for evaluation of past performance. Proposals must be received no later that 05 Mar 04 at 3:00PM (local time.
