- 12 -- COOLER ASSEMBLY W31P4Q-04-R-0217 AMSAM-AC-LS-MC, VIVIAN JENNINGS, CONTRACT SPECIALIST, 256-842-6115. *****ATTENTION*****FORMAL COMMUNICATION RELATED TO THIS ACQUISITION SHOULD BE DIRECTED TO VIVIAN JENNINGS, AMSAM-AC-LS-MC, CONTRACT SPECIALIST, 256-842-6115.    YOU MAY FAX OR MAIL YOUR REQUEST. **TELEPHONE REQUEST WILL NOT BE HONORED** (EMAIL:    vivian.jennings@redstone.army.mil    ) SYSTEM: BFVS (BRADLEY), PRON: D14V0193, Nomenclature: Cooler Assembly, NSN:    1240-01-509-7172, PN:    3271055-0401.    This proposed acquisition is for a requirements contract which will be segregated into five (5) program periods with orders to be issued one or more times as requirements materialize.    However, this does not preclude the Government from ordering the maximum quantity at any time during the requirements period.    The known quantity is 264, and the maximum quantity is 572.    Destination to be furnis hed.    Delivery Schedule 100 days after contract award.    The closing date annotated is an estimated date and may be adjusted dependent upon the date of release of solicitation; however, solicitation will not close prior to the closing date stated above.    Of ferors must be approved and qualified.    The Government does not possess sufficient documentation to acquire these items from other than the qualified source.    A technical data package, which defines all engineering and quality requirements, is not availabl e.    This proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one, or a limited number of sources.    Suggest, that small business firms or others interested in subcontracting opportunities in connection with this procurement make contact with the firm(s) listed.    Foreign firms are reminded that requests for solicitation must be processed through their respective embassies.    Request for proposal will be issued to Carleton Life Support Systems    Inc., 2734 Hickory Grove Road, Davenport, IA 52804-1299 (Cage Code 99251).    All responsible sources may submit a bid /proposal, or quotation, which shall be considered by this agency, request solicitation package from address above, ATTN: AMSAM-AC-LS-MC, FAX Number 256-955-7737.    The most recent representative unit price and quantity relative to this acquisition is NSN: 1240-01-L11-5428, Award date: 01 Nov 2002, Qty: 2 each, Unit price: $36,879.00.    The Government provides this price history without assumi ng the responsibility for any resulting conclusions or interpretations that may be made.    The price history is provided for informational purposes only and should not be relied upon as a basis for offer/bid.
