- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being accepted and a written solicitation will not be issued. This proposed action is for services for which the Government intends to solicit and negotiate with one source VISX Incorporated, under authority of FAR 6.302-1, Only One Responsible Source and No other Supplies or Services Will Satisfy Agency Requirements. Interested persons may identify their interest and capability to respond to the requirement. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Governm ent. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This Solicitation M6700104Q0017 is a Request for Quote to provide patient treatment cards (keycards) to be used with VISX Star 3 Laser System. The requirement is for one-year period with provisions for 4 one-year option periods. This solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular 2001-14, effective June 23, 2003. The applicable North American Industry Classification System code is 339112 and the corresponding size standard is 500. Offerors must be registered in the Central Contractor Registration (CCR) database. Lack of registration in the CCR will make an offeror ineligible for award.
