- NA The response date for offerors has been extended from 30 Jan 04 to 6 FEB 04. This is a nonappropriated fund purchase and it does not obligate the military community through the sale of goods or services and the collection of fees and charges for participation in military community programs. This purchase does not involve federal tax dollars. The response date for offerors has been extended from 20 Jan 04 to 30 Jan 04. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are requested, attachments to this announcement are included for completion. This solicitation, F33601-04-T-0012, is a Request for Quotation (RFQ). This request is for a concessionaire contract for a Hair Styling Salon located at the Club and Banquet Center (building 280, Area C) at Wright-Patterson AFB (Dayton, OH). This is a one year agreement with four, one- year options. The attachments include a cover letter, Appendix A "Statement of Work"(with appendices B and C), the concessionaire agreement (to complete and return), Instructions to Offerors, Evaluation of Offers, Special Provisions, General Provisions, Representations and Certifications (to complete and return), and a performance questionnaire (to complete and return). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-06. The offeror shall return a copy of FAR 52.212-3 (Representations and Certification). Interested parties are required to submit their quotes along with proof (performance questionnaires) that their qualifications meet the specifications set forth in this synopsitation to Jewell Michaels, 88 ABW/PKSB, 1940 Allbrook Drive, Suite 3, Wright-Patterson AFB OH 45433-5309 by 2:00 PM EST on 20 Jan 04.
