VENDORS THAT HAVE RESPONDED TO THE ORIGINAL POSTING NEED NOT RESPOND AGAIN. YOUR SUBMISSION WILL BE INCLUDED AS PREVIOUSLY SUBMITTED. Potential sources sought to participate in the regional standardization of Safety Blood Collection Needles & Tube Holders. TRICARE Regions 9 and 10, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing Safety Blood Collection Needles & Tube Holders. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) for Safety Blood Collection Needles & Tube Holders , your company is eligible to participate in the standardization process. Please respond by Email: cingrao@reg9.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than February 5, 2004. For questions, please call (619) 532-8517. 1. Does your company manufacture safety blood collection needles? 2. Does your company manufacture tube holders? If yes, what type(s) (safety, non-safety, single-use, multi-use, etc?)? 3. Are your products available through the Prime Vendor, Owens & Minor? 4. What is your DAPA Number? 5. Under what brand(s) does your company manufacture or distribute your product? 6. Are your products FDA approved? 7. Is the safety feature an integral part of the safety blood collection needle? 8. Does the safety feature activate automatically or does it have to be activated by the user? 9. Does the safety feature require the user?s hands to remain behind the needle at all times? 10. After activation of the safety feature, does it remain activated during disconnection from the patient and/or blood collection tube and after disposal? 11. At what point during use is the safety feature activated? 12. Is the device capable of multi-tube drawing, down to a 4 ml tube? 13. Is the device capable of adapting for use with pediatric tubes? 14. Does your company have butterfly needles with a safety feature? If so, what is the length of the extension tubing? 15. Are your products latex free? 16. Are your safety needles radio-opaque? 17.
