- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, F13MED33210100, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-19 and Defense DCN 04-01. The solicitation listed here is 100% small business set aside. The Federal Supply Class (FSC) is 3540 The associated Standard Industrial Classification (SIC) is 3845. The North American Industry Classification System is 334510 and the associated business size standard is 500. The requirement is for total maintenance service agreement for VISX laser eye surgery equipment, serial number 5147. Travis AFB intends to award sole source contract to VISX Incorporated. All responsible resources may submit a bid, proposal, or quotation, which shall be considered by the agency. To view requirements an electronic request for Statement of Work (SOW) must be provided. Quotes may be transmitted by e-mail or FAX to (707) 424-2712 and are to be received at the 60th Contracting Squadron no later that 4:30 PST on 30 June 2003.
