Description
COMBINE SYNOPSIS/SOLICITATION FOR Mock JCAHO Survey This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Relations document (FAR) subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. Solicitation number FA4600-04-T-0009 and is being issued as a Request For Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-18. This electronic announcement constitutes the only solicitation being issued. This procurement is set aside 100% for small business concerns. The North American Industry Classification System (NAICS) code is 541611, and the small business size standard is $ 6 million annually. The Government intends to issue a firm fixed price purchase order for line item 0001 Non-Personal Services to perform a Mock Joint Commission Accreditation of Healthcare Organizations (JCAHO) survey for the 55th Medical Group, located at Offutt AFB, NE. GOAL: TO PROVIDE SERVICE FOR CUSTOM JCAHO MOCK SURVEY. CONTRACTORS PERFORMANCE PERIOD: FEBRUARY 24-27, 2004. CUSTOM JCAHO MOCK SURVEY STATEMENT OF WORK JOINT COMMISSION ON ACCREDITATION OF HEALTHCARE ORGANIZATIONS (JCAHO) CUSTOM MOCK SURVEY. I. INTRODUCTION The 55th Medical Group-Ehrling Berquist Hospital requires the non-personal services of a contractor to provide assistance in the preparation of the July 2004 JCAHO survey. Our Average patient admission per month over past 12 months is: 147.75. Behavioral Health is not available on an inpatient basis. The hospital contains most services. We do have a satellite pharmacy on the base proper. Flight Medicine is in a building on base, along with Public Health and Bioenvironmental Engineering. The Health & Wellness Center (HAWC) (Physical Therapy has a satellite office in the HAWC) and the Mental Health (Life Skills) Clinic are also on the base proper. Actual care is provided in Flight Medicine, the PT office in the HAWC and Mental Health as entities away from the hospital. The Mock Survey dates were chosen to prevent a conflict with the ORE (Operational Readiness Exercise) and ORI (Operational Readiness Inspection) schedule. II. SCOPE OF WORK The work involves a broad spectrum of functional reviews, on-site educational assistance and consultative services. All such work shall be executed in an advisory and assistance capacity, and to include, but not limited to the following: Integrated Hospital Consultation Assess the patient rights function including organization ethics. Review the implementation and documentation of the patient assessment function including initial assessment, pathology and clinical laboratory services, reassessment, care decisions, supporting structures and additional requirements for specific patient populations. Evaluate the mechanisms for the provision of care including: planning and providing care, anesthesia care, medication use, nutrition care, operative and other procedures, rehabilitation services and special treatment procedures. Assess the patient and family education and responsibilities to include identification of and response to special educational needs. Evaluate the continuum of care function. Evaluate the performance improvement function including the organization's processes for planning, measuring, assessing and improving patient care. Evaluate the leadership function including: planning, directing and integrating services, as well as the role leadership play in evaluating improved performance. Assess Life Safety Code issues and the environment of care function including documentation of the design, implementation, measurement of outcomes, the social environment, and provisions of the Statement of Conditions. Assess the human resource management function, including; human resource planning, orientation, training, education, competence assessment and managing staff requests. Evaluate the information management function including planning, patient specific data and information aggregate data and information, knowledge-based information and comparative data and information. Evaluate the processes used for surveillance, prevention and control of infection. Evaluate the medical staff structure including processes for appointment, reappointment, and credentials review and privileging of the medical staff, as well as integration of performance improvement data into these processes. Assess the governance, management and nursing leadership structures. Educate the medical and clinical support staff relative to improving organizational performance standards. Assist in the development of strategic plans for improving organizational functions. Behavioral Health Care (Alcohol and Drug) Consultation Assess the performance improvement activities, including the flow of information. Provide an educational update on the elements of quality assessment and improvement. Evaluate the written plan for professional services and staff composition. Evaluate the following elements with clinical patient services (focusing assistance on areas of difficulty identified): -Direction; -Education; -Staffing; -Policies and procedures; -Records and documentation; and -Infection control. Assess the professional staff organization, including credentialing and privileging processes. Assess the therapeutic environment and patient rights issues. Evaluate patient management processes, including intake assessment, treatment planning, progress notes, discharge summaries, aftercare, and special treatment procedures. Assess the utilization review and infection control programs. Assist in the development of strategic plans to improve or enhance the areas assessed. III. CONTRACTOR QUALIFICATIONS: Current, qualified and experienced JCAHO surveyors Expertise and knowledge in the interpretation and intent of 2004 and new for 2005 standards in the Comprehensive Accreditation Manual for Hospitals; Skilled in providing a collaborative and educative approach to standards review and the mock survey process. Demonstrated ability to identify facility-specific methods for demonstrating compliance with applicable standards. Cross-trained in more than one specialty area (i.e. Nurse surveyor cross-trained in Behavioral Health). This criterion is desired but not required. IV. DELIVERABLES Pre-consultation: Assistance from the lead consultant to develop an agenda customized to the facility and a pre-consultation review of key written plans and policies by each consultant. Provision of a sample agenda. Education: Educational sessions for the medical and clinical support staffs, as well as education provided with each individual interview to familiarize staff with the JCAHO standards and survey process. Assessment: To include: -- Meeting with the leadership team to assess the leadership functions. -- Patient unit or clinic site visits to assess patient care activities. -- Review of medical record documentation with staff to identify areas for improvement. -- Meetings with cross-functional teams to assess the improving organizational performance function. -- Work sessions with individuals/multidisciplinary groups to evaluate patient care and organizational functions. -- Building tour and review of management plans and Statement of Conditions to assess the environment of care function. Recommendations: Recommendations will be made in all areas to assist staff with developing processes and methods to effectively and efficiently comply with JCAHO standards. Planning: At the conclusion of the "Mock Survey" provide an on-site report of findings and recommendations for action to key staff. Report: A written summary of consultation services, findings with reference to JCAHO and strategies for corrective action and follow-up actions. V. GOVERNMENT -- FURNISHED ITEMS The government will provide an office space, parking space, office equipment and necessary supplies to complete the survey.
