Description
This is Modification 1 to the combined synopsis/solicitation for FA2816-04-T-0003. Modification 1 encompasses the following information: (1) The sentence ending with the attached Statement of Work, is hereby deleted. (2) The following information is replacing the Statement of Work and will be incorporated in the description of work: (a) Delivered finished articles shall conform to the generally accepted industry standards of quality cleanliness, finish, and appearance. (b) Packaging. Clean laundry is to be sealed in cellophane plastic wrap. Bed linens and small towels shall be packaged in bundles of 20 each. Bath towels will be packaged in bundles of 10. (c) Clean linens shall be stored and transported in a separate and clean area of the delivery vehicle, totally separate from potential contamination by soiled linens. (d) The contractor shall furnish delivery tickets to be used for recording the pickup and delivery of linens. (e) Quantities delivered shall match the quantities specified on the delivery ticket. (f) The contractor shall respond to a service request within four hours when quantities delivered do not match and are less than delivery tickets amounts. (g) The government shall furnish 75 queen and 30 twin bedspreads. (h) The contractor shall furnish all other linens. (i) The contractor is responsible for compliance with all Municipal, County, State and Federal laws, rules, ordinances, and regulations applicable to the business performed under this contract. (j) Pickup and delivery shall be Monday, Wednesday, and Friday between the hours of 8:30 and 11:30 am, excluding federal holidays when the contractor shall adjust the schedule accordingly. (k) The contractor shall obtain all necessary card, passes, decals, or other items required for access to the area in which the work will be performed before beginning service. (3) The statement on the combined synopsis/solicitation: The list of appropriate provisions and clauses are attached is hereby deleted. (4) The completed RFQ with the required attachments shall be received by 12:00 noon, Pacific Standard Time, 29 December 2003, at 61 CONS/LGC. They will be accepted by mail at: 61 CONS/LGC, ATTN: TSgt Elke Beauregard, 2420 Vela Way, Suite 1467, El Segundo, CA 90245-4659. RFQs will also be accepted by FAX to: 310-363-1312.
