Description: Medical Doctor to provide hands-on physical examinations for Government employee population, as well as serve as Medical Officer for Occupational Health Unit. The hands-on, physical exam can be accomplished in the fully functional, staffed clinic on -site, or at a facility within a ten-mile radius of the US Army Corps of Engineers Office located at 109 St. Joseph Street, Mobile, AL. Medical Doctor who has been professionally employed in the practice of Occupational Medicine for a minimum of one (1) y ear. Medical Doctor will complete the physician portion of the employee physical examination and provide the completed record to the Occupational Health Unit for appropriate maintenance in the employees medical record. The record will be maintained on go vernment forms, which are provided. NAICS Code is 621111 with a size standard of $7.5 million. This is total set aside for small business concern. The solicitation will not be provided in a printed-paper format; however, the Government reserves the righ t to revert to paper medium when it is determined to be in the best interest. Therefore, Offerors are required to self-register their firm or office on the Internet and download the solicitation/specifications at the following Internet Site: http://www.s am.usace.army.mil/sam/ct. NO TELEPHONE, FAXED OR MAILED REQUEST WILL BE ACCEPTED. Those registering are responsible for the accuracy of the information on the mailing list. Registration must include the firm's physical address, telephone, a valid fax numbe r and a point of contact. All notifications of changes to this solicitation shall be made through the Internet. Therefore, Offerors who have registered are required to review the Internet daily for any posted changes to the solicitation.
