- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being accepted and a written solicitation will not be issued. This proposed action is for services for which the Government intends to solicit and negotiate with one source VISX Incorporated, under authority of FAR 6.302-1, Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. Interested persons may identify their interest and capability to respond to the requirement. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Governmen t. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This Solicitation M6700103Q0005 is a Request for Quote to provide maintenance service on government owned VISX STAR Excimer Laser System and VISX WaveScan Wavefront System. The requirement is for a one-year period, 13 Dec 03 thru 12 December 04 This solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular 2001-14, effective June 23, 2003. The applicable North American Industry Classification System code is 334510 and the corresponding size standard is 500. Offerors must be registered in the CCR (Central Contractor Registration) database. Lack of registration in the CCR will make an offeror ineligible for award.
