Description
NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. Solicitation number W91248-04-Q-0004 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisiti on Circular 2001-17. Acquisition is set aside for small business concerns. The NAICS Code is 711130 and the small business size standard is $6,000,000. Performance period is for one (1) 12-Months Period and anticipated start date is on or about 1 Novemb er 2002. The provision at 52.212-1 Instructions to Offerors-Commercial Items is applicable and is addended as follows: Paragraphs (b)(5), (d), (e), (h) are deleted and (b)(10) is revised to state, 'Resume to include qualifications and experience in the ar ea(s) which you are submitting an offer for'. This provision is further addended to incorporate 52.252-1 Solicitation Provisions Incorporated by Reference (Fill-in information: http://acqnet.saalt.army.mil/library/default.htm); 252.209-7003 Compliance wit h Veterans Employment Reporting Requirements. The provision at 52.212-2 Evaluation-Commercial Items is applicable; Price and Qualifications/Experience are the evaluation criteria in Paragraph (a). Price is slightly more important than Qualifications/Expe rience. The Government intends to make multiple awards to the responsive and responsible offerors whose offers are most advantageous to the Government considering price and non-price related factors. The Government reserves the right to conduct auditions /interviews with participants. Offerors are instructed to submit completed copies of FAR Provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with offers. OFFER SCHEDULE MUSICIANS FOR RELIGIOUS SERVIC ES UNIT OF UNIT TOTAL CLIN DESCRIPTION QUANTITY ISSUE PRICE PRICE 0001 Provide musician Services for 9:00 A.M. 124 each $ $ Collective Protestant Service at Grace Chapel. This includes rehearsals, memorial services, and ceremonies. 0002 Provide musician Services for 8:00 A.M. 171 each $ $ Collective Protestant Service at Gospel Congregation. at Memorial Chapel. This includes rehearsals, memorial services, and ceremonies. 0003 Provide musician Services for 11:00 A.M. 124 each $ $ Protestant Congregation at Soldiers Chapel. This includes rehearsals, memorial services, and ceremonies. NOTE: *QUANTITIES LISTED ABOVE ARE ESTIMATES ONLY; HOWEVER, THESE QUANTITIES SHALL NOT BE EXCEEDED WITHOUT PRIOR APPROVAL OF THE CONTRACTING OFFICER. PAYMENT WILL BE MADE ONLY FOR ACTUAL QUANTITIES USED. PERFORMANCE WORK STATEMENT MUSICIAN SERVICES FOR RELIGIOUS SERVICES INSTALLATION CHAPLAINS OFFICE FORT CAMPBELL, KENTUCKY C.1. GENERAL: C.1.1 Scope of work: Provide music coordinator and musician services for Catholic and Protestant religious services at various chapels at Fort Campbell, Kentucky. These services will also be required for rehearsals, memorials, and ceremonies. Contractor shall maintain a professional decorum at all times. Worship services are a statutory (Title 10) Essential Element of Religion. The contractor falls under the Chaplain??????s Religious Support Program as defined in the Commanders Master Religious Program . The mission of the Chaplains program is to provide for Essential Elements of Religion to include worship services for all assigned soldiers, civilians, and family members. As a non-personal services contractor, the musician is not an employee of the Ch apel or Installation Chaplains Office staff. C.1.2 Successful offerors shall possess the skills, knowledge, and training to satisfactorily perform the services required by this contract. Personnel performing work under this contract will not be considered employees of the Government. The basis for awarding the musician contracts is job experience. C.1.3 The Contracting Officer may require removal from the job any employee working under this contract for reasons of misconduct, security, or found to be or suspected to be under the influence of alcohol, drug or other incapacitating agent. Personnel sh all be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary in the interest of the Government. The Government will provide the Contractor with a detailed written explanation of the Government?? ????s investigation and findings, and reason for the demand to remove the employee from the job, immediately upon conclusion of the investigation or within five business days in the event that investigation is not conducted. The installation commander has the authority to bar individuals from the installation. C.1.4 Contractor personnel and property are subject to the same laws and regulations pertaining to law enforcement, searches, and seizures as other personnel on the installation. C.1.5 GATE CONTROL. Fort Campbell is a closed post. Unscheduled gate closures may occur at any time. Personnel and vehicles entering or exiting the installation may experience delays. All vehicles and personnel are subject to search. C.1.6 VEHICLE REGISTRATION, LICENSING, AND OPERATION. Proper state registration, proof of insurance and a valid drivers license are required to operate a privately owned vehicle on post. Registration with the Provost Marshals Office is required. Decals shall be returned to the Provost Marshals Office within 3 business days after termination or completion of this contract, when state license is no longer valid, or upon sale of the vehicle. C.1.7 WORK HOURS: Work hours and rehearsals vary. See the times services are required for each CLIN on the Offer Sche dule. Rehearsals are required approximately once weekly and last approximately 60 to 90 minutes. Worship services for both Catholic and Protestant services each weekend last approximately 60 to 90 minutes. We estimate approximately 10 memorial services/ ceremonies will be performed during this 12-month performance period. However, these are estimates only and may require adjustment. C.2 DEFINITIONS: C.2.1 CONTRACING OFFICER. A person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. C.2.2 CONTRACTING OFFICERS REPRESENTATIVE (COR). An individual designated in writing by the Contracting Officer to perform specific technical or administrative functions. The COR is not authorized to make any commitments or changes that will affect price , quality, quantity, delivery, or any other term or condition of the contract. C.3 GOVERNMENT FURNISHED PROPERTY. C.3.1 The Government will furnish facilities, property, equipment and materials used by the contractor during performance of the music coordinator/musician services at Fort Campbell Chapels. C.3.2 Equipment. The Government will provide musical equipment and materials required for these services. C.3.3 Emergency Medical Care. The Government will provide emergency medical care for contractor personnel injured on-the-job or experiencing emergency illness at Blanchfield Army Community Hospital (BACH). Emergency medical care will be limited to those services necessary to prevent undue suffering or loss of life. Emergency care provided to contractor personnel will be priced at the established Army rate and the contractor shall provide reimbursement to the Government. C.4 CONTRACTOR FURNISHED ITEMS: C.4.1 Contractor personnel shall be qualified, knowledgeable and experienced in providing music coordinator/musician services for religious services. C.5 SPECIFIC TASKS MUSICIAN SERVICES (CLINs 0001 THROUGH 0004) C.5.1 Provide appropriate musical support during the designated worship services. C.5.2 Provide all musical accompaniments during the worship services to include congregational music, offertory, prelude, and postlude music. C.5.3 Accompany (play piano/organ/guitar/keyboard) or direct musical selections during designated worship services including accompany singing of hymns, choruses, and spiritual songs; choral responses, anthems, and special music that enhances the atmospher e of worship for the particular congregation. C.5.4 Accompany choir rehearsals: play the piano/organ/guitar/keyboard and/or direct accompaniment for choir/worship team rehearsals. C.5.5 Communicate with the chaplains, priests, song/worship leaders, and other musicians conducting the service to ensure a coordinated service of worship for the congregation. Plan and coordinate all musical selections with the Chaplain/Pastor of the con gregation or appropriate designated representative to select music in keeping with the theme of worship service. C.5.6 Select for choir/solo presentation music that enhances the atmosphere of worship for the particular congregation. C.5.7 Coordinate and/or provide music for all services and adjust tempo and style of choir selections to enhance the congregations ability to participate in worship. C.5.8 Provide music selections to the Chaplain/Pastor by the time designated for publication in worship service bulletins. C.5.9 Provide music for memorial services/ceremonies. C.5.10 Musician(s) will be responsible for submitting invoices for the previous months contract services to the Chaplaincy Resources Manager, Bldg. 3101, for verification no later than (NLT) the 10th working day of the month. C.5.11 Musician(s) will be responsible for finding and paying substitute musician(s) services if they cannot perform the weekly service themselves.
