- Description
- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, 644-10-04, is issued as a Request for Proposal (RFP). Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-16. The North American Industry Classification System Number (NAICS) is 485991 and the Business Size Standard is $6.0M. The proposed contract listed here is Set Aside: UNRESTRICTED. NON-PERSONAL SERVICES: The Contractor shall furnish all labor, tools, vehicles, personnel, management, supplies, equipment, and transportation and reports necessary to provide Ambulette/Medical Coach (stretcher) and Wheelchair Van Services to beneficiaries for the Carl T. Hayden VA Medical Center, Phoenix, Arizona. The primary intent of this contract is for pre-arranged, non-emergency, lie-down transportation or wheelchair van services. NOTE: This contract is not for emergency medical services. Anticipated award date is 1 Jan 2004 and performance date is 1 Jan 04 through 30 Sep 04, with 4 option years. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition in accordance with the specifications. The provisions at 52.212-2, Evaluation - Commercial Items, applies to this acquisition with the blanks filled in as follows: The following factors shall be used to evaluate offers: Past Performance, Software and Equipment offered and Price and Price related factors. Offers shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. The clause FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. Additional FAR 52.212-5 clauses cited in the clause applicable to this acquisition are (b) 1, 7, 8, 13?19, 22, 24-26, 30, (c) 1, 2, and 3. The following clauses are also applicable to this acquisition: 52.203-3 Gratuities, 52.204-4 Printing/Copying Double-Sided on Recycled Paper, 52.209-6 Protecting the Government?s Interest When Subcontracting with Contractors Debarred, Suspended, or Proposed for Debarment, 52.232-17 Interest, 52.242-13, Bankruptcy, 52.228-5 Insurance-Work on a Government Installation, 52.237-2 Protection of Government Buildings, Equipment, and Vegetation; VAAR clauses: 852.270-1 Representatives of Contracting Officers, 852.270-4 Commercial Advertising, and 852.237-70 Contractor Responsibilities. Information required in blanks immediately follows the clause/provision: 52.217-8 (Nov 1999), Option to Extend Services 15 days; 52.217-9 (Mar 2000), Option to Extend the Term of the Contract (a) 15 days, at least 60 days, (c) 9 months and 4 years.
