Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation and a blanket purchase agreement (BPA) will be awarded upon review of quotes. Quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation reference number is FA4418-04-Q-0001 and is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-16. THIS ACQUISITION IS UNRESTRICTED. The associated NAICS code is 621512 with a $11,500,000.00 size standard. Offerors shall include all costs on the attached price schedule, Attachment 1. All responsible sources may submit a quotation, which shall be considered by the agency. STATEMENT OF WORK FOR RADIOLOGICAL INTERPRETATIONS FOR THE 437TH MEDICAL GROUP. The contractor shall: (1) Provide courier service to pick up films Monday ? Friday, 7:30 AM ? 4:30 PM from the 437th Medical Group, Diagnostic Imaging Dept., Charleston AFB, SC. Perform plain film interpretation, wet read, and B-read studies when required. Return film to the Radiology Dept. (2) Transcribe and verify the reports in CHCS (Composite Health Care System). This system is accessible through the internet. The Navy Systems Office, CHCS can help with getting online for this capability. (3) Call the providers or Diagnostic Imaging Dept. when the radiologist locates serious or life threatening results. If the provider is not available, any doctor in the department can take the call. The 437th Diagnostic Imaging Dept. will provide the radiologists with a phone listing of the providers. (4) Use board certified radiologists currently licensed in the state of South Carolina to real all films. (5) Provide a current copy of the credentials for each certified radiologist to the 437th Medical Group annually. FAR Parts 12 and 13 evaluation procedures will be used. A blanket purchase agreement will be written for a period of five years beginning 23 Dec 03. Offerors shall provide as a response to this solicitation: (1) a completed price schedule,(2) completed FAR Clause 52.212-3, and (3)CAGE Code.
