- Provide all personnel, equipment, and supplies necessary for certified home health aid services for the John D. Dingell Department of Veterans Affairs Medical Center, 4646 John R. Street, MI 48201. Contractor shall perform these services for patients residing in Detroit and Wayne, Oakland, and Macomb Counties. The North American Industry Classification System (NAICS) for this procurement is 621610, Home Health Care Services. Projected period of service is from date of award through December 31, 2004, with two pre-priced option years. Request for Proposal is now available on this website. Requests for proposals may also be submitted in writing to the John D. Dingell Department of Veterans Affairs Medical Center, Acquisition Management (001R-A), 4646 John R. Street, Detroit MI 48201. Telephone or internet requests will not be accepted. All requests must reference solicitation RFP 553-17-04. Proposals are due by 4:00 pm, local time, Friday, December 12, 2003, in the Acquisition and Material Management Office (001R-A), (Room B-4213 if hand carried), John D. Dingel Department of Veterans Affairs, 4646 John R. Street, Detroit, MI 48201. Proposals received after the due date will not be considered. It is the contractor?s responsibility to verify that receipt of any requests or documents have been received in the contracting office on time. It is the contractor's responsibility to assure requests are in writing, in a timely manner, and all information is correct.
