- Contractor must provide the full time services of one (1) Certified Registered Nurse Anesthetist (CRNA) for and at the Edward Hines, Jr. VA Hospital, Fifth Ave. & Roosevelt Rd., Hines, IL 60141; for the period of one year from the date of the award in accordance with all specifications, work statement, terms and conditions contained therein (with one one-year options exercised at the government?s discretion). This is set-aside 100% for small business with NAICS 621999 ($8.5 Million). Electronic version and hard copy of solicitation should be available on or about November 5, 2003. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
