Description
Sources having the ability to provide OUT-PATIENT assessment and counseling services in the Michigan City, Indiana area for the drug abuse treatment of male and female Federal offenders (inmates) held under the authority of various United States Statutes. Performance period under this contract will be for a one year base period to begin on October 1, 2004 with four (4) one year option periods, with service ending (if all option years are renewed) on September 30, 2009. The services required for this contract include: Base Period: October 1, 2004 to September 30, 2005. Estimated Number of Inmates 15. (15) Intake Assessments (one per inmate); Individual Counseling Sessions (one hour session twice a month per inmate) 48 Individual units x 15 inmates = 720 total units; Group Counseling Sessions (one and half hours sessions weekly per inmate) 144 Group Units x 15 inmates = 2,160 total units, Option Year One: October 1, 2005 to September 30, 2006. Estimated Number of inmates 15. (2) Intake Assessments (one per inmate); Individual Counseling Sessions (one hour session twice a month per inmate) 48 Individual units x 15 inmates = 720 total units; Group Counseling Sessions (one and half hours sessions weekly per inmate) 144 Group Units x 15 inmates = 2,160 total units. Option Year Two: October 1, 2006 to September 30, 2007. Estimated Number of inmates 16. (6) Intake Assessments (one per inmate); Individual Counseling Sessions (one hour session twice a month per inmate) 48 Individual units x 16 inmates = 768 total units; Group Counseling Sessions (one and half hours sessions weekly per inmate) 144 Group Units x 16 inmates = 2,304 total units. Option Year Three: October 1, 2007 to September 30, 2008. Estimated Number of inmates 17. (6) Intake Assessments (one per inmate); Individual Counseling Sessions (one hour session twice a month per inmate) 48 Individual units x 17 inmates = 816 total units; Group Counseling Sessions (one and half hours sessions weekly per inmate) 144 Group Units x 17 inmates = 2,448 total units. Option Year Four: October 1, 2008 to September 30, 2009. Estimated Number of inmates 18. (6) Intake Assessments (one per inmate); Individual Counseling Sessions (one hour session twice a month per inmate) 48 Individual units x 18 inmates = 864 total units; Group Counseling Sessions (one and half hours sessions weekly per inmate) 144 Group Units x 18 inmates = 2,592 total units. The services described shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work, dated November 16, 1999. The estimated closing date is o/a November 17, 2003. All responsible sources may submit an offeror which will be considered by this Agency. The RFP 200-0801-NC will be posted o/a October 16, 2003.
