- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are requested. Solicitation number RMK0E030081 is applicable and is issued as a Request for Quote (RFQ). The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition 97-26. The requirement is full and open with a NAICS Code of 424430 and the business size maximum is $12.0 million. The description of the service in Clin 0001 is: Delivery of Dairy Products to Chemawa Indian School in Salem, Oregon. The contractor shall provide a price list of all the different dairy products including milk that can be delivered to Chemawa Indian school, also include the unit in which these products can be purchased at. Any award resulting from this request for quotations will be made to a responsive, responsible quoter using a OF 349 form. Quotes will be evaluated on a best value basis. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this provision is applicable, in which the term offeror, whenever it appears, shall be replaced with the term quoter. Quoters are required to submit completed copies of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes along with a DUNS Number. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statures or Executive Orders, Commercial Items, applies to this acquisition. The following clauses within 52.212-5(b) are applicable to this acquisition: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33. The following clauses are also applicable to this acquisition: 52.222-6, 52.222-7, 52.222-8, 52.222-9, 52.222-10, 52.222-11, 52.222-12, 52.222-13, 52.222-14, and 52.222-15. Required Central Contractor Registration and FAR clauses 52.219-6, Notice of Total Small Business Set-Aside and 52.237-2, Protection of Government Buildings, Equipment and Vegetation FAR 52.242-15 Stop Work Order, FAR 52,242-17 Government Delay of Work, Department of the Interior Acquisition Regulation (DIAR) 1452.226-70 Indian Preference, DIAR 1452.226-71 Indian Preference Program, DIAR 1452.204-70 Release of Claims are applicable.
