Description
This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is F39601-03-Q-C109 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2001-15, Defense Federal Acquisition Regulation Change Notice (DCN) 20030720 and Air Force Circular (AFAC) 2003-0501. For information purposes the Standard Industrial Classification Code is 5941. The North American Industry Classification System is 451110. The BID SCHEDULE shall be for the purchase, delivery and installation on site of the following SkateWave equipment or equal: one 14? round kinked grind rail; one 14? combo track rail; one 2? bank jump ramp; one 3-sided pyramid with curb rail, 4? by 4? bank to bank; one combination 4? by 8? quarter pipe next to a 5? by 12? quarter pipe; one combination 3? launch box next to a 4? launch box; one combination 4? by 12? quarter pipe next to a 5? by 8? quarter pipe; and one standard park rules sign with surface mount posts. The price must include delivery and on-site installation to provide a ready-to-use skate park. The contractor must install equipment per the manufacturers recommendations and work in a timely and safe manner. The site where the equipment must be installed is a rubber-coated asphalt tennis court, 55? wide by 120? long. The contractor must provide 3 distinct professional design layouts for the park within these parameters, including appropriate safety borders, which the government will have the choice between without affecting cost. The contractor must provide separate delivery and installation schedules that are realistic for the contractor to achieve. The site is located on Ellsworth AFB, SD and the contractor may schedule a site visit (if desired) by contacting the Contracting Officer at 605-385-1734 in sufficient time to meet the quote suspense listed herein. Offerors must provide all specifications and drawings if bidding an equal item. Contractor will provide personnel to furnish all labor, tools, parts, materials, and equipment necessary to deliver and professionally install the equipment per the manufacturers recommendations. FOB Point: Destination. Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF OFFERS; NAME, ADDRESS; TELEPHONE NUMBER OF OFFEROR; TERMS OF THE EXPRESSED WARRANTY; PRICE, ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Quote shall also contain all other documentation specified herein. Signed quotes must indicate quantity, unit price and total amount for each item. Offerors shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. PERIOD OF ACCEPTANCE OF OFFERORS: The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. LATE OFFERS: Request for Quotations or modification of quotes received at the address specified for the receipt of offers after the exact time specified for receipt of offers WILL NOT be considered. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order resulting from this request for quotation to the lowest priced offer. This acquisition is set-aside exclusively for small business concerns. All offerors MUST BE REGISTERED IN THE CENTRAL CONTRACTOR REGISTRATION (CCR). Offerors that fail to furnish the required representation information, or reject the terms and conditions of the solicitation, may be excluded from consideration.
