Description
NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. The Pacific Regional Contracting Office (PRCO), Tripler Army Medical Center (TAMC), Hawaii has a requirement for the maintenance and service of the water supply for the Hitachi analyzer, weekly service for glassware water lab, and quarterly service for ext raction lab located at Tripler Army Medical Center, Hawaii. The contractor shall provide all necessary trained experienced personnel, labor, tools, diagnostic equipment and software, hardware, material, supplies, transportation, parts and equipment necess ary to maintain the systems in a fully functional state. Contractor shall provide (1) routine service call weekly during normal working hours, 7:30 a.m. to 4:00 p.m., Monday through Friday. In case of emergency, the Contractor must respond telephonicall y within two (2) hours, on-site within four (4) hours of notification, 24 hours a day, 7 days a week. System must be made operational within eight (8) hours of notification. The Contractor must have deionozation regeneration facilities located on the isl and of Oahu to insure adequate supply and immediate turn around time of D.I. tank. The Contractor must monitor all equipment and instruments and change out tanks if needed. The Contractor must perform the following tests: City/Products TDS, Make-up and polish resistivity, check ultra-violet sterilizer, chlorine, PH 5.5 to 6.5, feed water temperature, and replace expendable parts if needed. Equipment list: (3) modulab polishers, (2) mixed bed 350 feeding, (2) Hitachi analyzers (no change), and (1) feed ing washroom system. The rental fee of three (3)360 cubic foot tanks should be included. The Contractor must provide a detailed written report of every routine and/or emergency service performed to the staff person on site of Tripler AMC, HI 96859-5000. PRCO anticipates an award of a firm fixed-price contract resulting from the Request for Proposal (RFP). Offers will be evaluated based on technical capability/experience of the item offered to meet the Government requirement; past performance and price ( see Statement of Work in RFP and provide the appropriate documentation pertaining to the requirement. The NAICS code for this procurement is 333319 and the annual size standard is 500 employees. The contract term will be a base period of one year to commence on or about 01 October 2003 - 30 September 2004, plus four one year options. The total term of the contract, including all options, will not exceed 60 months. If the Government does not awar d a follow-on contract in time, the Government may require the Contractor to continue performance for an additional period up to six months under the Option to Extend Services clause. In either case, the Government will not synopsize the options when exer cised. The requirement is set-aside for Small Business.
