Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 under FAR Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is RFQ 249-03-39 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-15. This is a 100% set-aside for small business. The North American Industry Classification System (NAICS) code is 339111, with a size standard of 500 employees. The James H. Quillen VA Medical Center in Mountain Home, Tennessee has a requirement for a doubled wall sound room with inside dimension of 9ft 8 inches by 9 ft 8 inches. Specifications include two housings of 4” thick modular panels with a 4 inch airspace and recessed electrical (hospital grade) (UL listed). The quote must include installation and freight cost with an additional estimate to unload the room and place it in storage at the VA facility. The installation will be coordinated with the Contracting Officer Technical Representative (COTR) at a specified date. If the installation does not coincide with the delivery date, the contractor will be responsible for unloading the shipment and store in a location designated by the COTR. Therefore, the quote must include a price to return at a later date to complete installation. The Contracting Officer intends to award on a best value basis utilizing price and other factors (technical specifications and past performance) under FAR Part 13.106. Technical and Past Performance when combined is greater than price. The anticipated contract award will be a Firm Fixed Price supply contract. Prospective Offerors are cautioned to submit price quotes on the most favorable basis, since the Government may elect to award with out conducting discussions. Delivery to be made 30 days after receipt of order. Delivery location is Warehouse, James H. Quillen VA Medical Center, Building 205, Mountain Home, TN 37684-4000. Invoices shall be sent to: James H.
