- NA Subject Request for Quote is for Personal Services - Neonatal Physical Therapy and is being processed in as a request for services by a commercial health care company. It is being issued as an RFQ. Madigan Army Medical Center, Tacoma WA intends to establ ish a firm fixed-price contract for part time personal services that will provide on call services as needed, as well as, scheduled appointments throughout the course of the year for a base period beginning on or about 01 Oct 2003 through 30 September 2004 plus four option years. Performance of service will be in accordance with federal and local Medical Treatment Facility regulations and requirement standards of Professional services. Requires American Board of Physical Therapy certification, as well as, 2 years experience with Neonatal Physical Therapy.
