- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The North American Industry Classification System (NAICS) Code applicable to this solicitation is 421450, Medical, Dental and Hospital Equipment and Supplies Wholesalers, and the small business size standard is 500 employees. This is a full and open competition (unrestricted). The VA Healthcare Network Upstate New York at Syracuse, NY intends to purchase a Radionics RFG-3C+ Radiofrequency Lesion Generator from State of Art Medical Products, 1056 Randolph Avenue, Rahway, NJ, 07065. In addition, the following items are to be purchased concurrently: (a) 1 ea, SMK-5, 5cm Electrode Kit with Cable and Sterilizing Case, (b) 1 ea, SMK-10, 10cm Electrode Kit with Cable and Sterilizing Case, (c) 1 ea, SMK-15, 15cm Electrode Kit with Cable and Sterilizing Case, (d) 1 ea, RFK-10, 10 cm Electrode Kit with Cable and Sterilizer Case, (e) 1 ea, RFK-15, 15cm Electrode Kit with Cable and Sterilizing Case. All proposed components and configurations must be FDA approved. This solicitation document incorporates Federal Acquisition Regulation (FAR) provisions and clauses in effect through Federal Acquisition Circular 01-15 and VA Acquisition Regulation (VAAR) clauses as follows: 52.211-6 Brand Name or Equal; 52.212-1 Instructions to Offerors Commercial Items; 52.212-3 Offeror Representations and Certifications ? Commercial Items; 52.212-4 Contract Terms and Conditions ? Commercial Items; 52-212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items with (b)(13), (14), (15), (16), (17), (18), (19), (21), (24), (25) and (30) checked; and VAAR 852.270-4 Commercial Advertising. Offers to include completed copy of 52.212-3 Offeror Representations and Certifications ? Commercial Items. The provision at 52.212-2, Evaluation of Commercial Items, applies to this acquisition. The evaluation criteria stated in paragraphs a) through d)of this provision is as follows. (1) Technical capability of the item offered to meet the government requirement and (2) price. The system offered must meet all the salient characteristics specified as follows, multi-purpose, in that it is able to perform, a) deep brain lesions, b) trigeminal neuralgia, c) cordotomy lesions, d) facet lesions.
