Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quote is being requested and a written solicitation will not be issued. This request for quotation (RFQ) number 04DWEL000002 is for Preventative Maintenance Agreements on fitness equipment for two (2) NASA health center sites managed by Federal Occupation Health. The Period of Performance shall be for a basic period of one year with four (4) one-year option periods. The NAICS code is 811310 with a size standard of $6M. THE GOVERNMENT KNOWS OF ONLY ONE SOURCE, LEISURE FITNESS, LOCATED AT 316 MARKUS COURT; NEWARK, DELAWARE, 19713 CAPABLE OF SATISFYING THE REQUIREMENT DESCRIBED HEREIN AND INTENDS TO NEGOTIATE AN AWARD ON A SOLE-SOURCE BASIS UTILIZING SIMPLIFIED ACQUISITION PROCEDURES. The Preventative Maintenance shall be scheduled with the individual site and be provided on a quarterly basis according manufacturers guidelines for all equipment and to the following schedule: Clean the exterior of all equipment with mild detergent; adjust and lubricate equipment per manufacturer guidelines; inspect for loose, broken or worn out parts; tighten any loose bolts and replace any missing bolts or fasteners; vacuum inside of motor compartment and underneath all treadmills; wax the decks of treadmills per manufacturer guidelines; notify W/F Coordinator of needed repairs or replacement parts and provide a written quote for their repair; upon completion of the preventative maintenance on all equipment, the vendor shall complete a FOH Maintenance Log sign it and have a member of the fitness center staff verify the services were completed and sign the log. The vendor shall submit a copy of the signed maintenance log along with the invoice for payment to the project officer or designee for verification and to forward on for payment. On occasion Emergency Service is required. Response time for emergency calls should be within 48 hours of the call to visit and diagnose (if parts are required) or repair (if no additional parts are required). Emergency visits costs should be included in the cost of the preventative maintenance agreement. Emergency visits for repair if no additional parts are required, shall be included in the cost of the preventative maintenance agreement. Upon diagnosis, if parts are required a written quote for repair will be provided to the site staff during the visit. Prior to repair, the staff will get prior approval of the purchase request for repair. Repairs will be paid for via credit card outside this agreement. THE CONTRACTOR IS REQUIRED TO PROVIDE A REPLACEMENT PIECE OF EQUIPMENT TEMPORARILY FOR ANY EQUIPMENT THAT IS EXPECTED TO BE OUT OF SERVICE FOR MORE THAN ONE WEEK. The first NASA fitness center is NASA-HQ Fitness Center located at 300 East Street, SW, Washington, DC 20546. The NASA-HQ fitness center has an average of 15 emergency visits per year.
