- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) F64133-03-T-0077 is being issued as a Request for Quote (RFQ). (iii) This combined synopsis/solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 01-14. (iv) This acquisition is a total Small Business Set-Aside. (v) CLIN 0001 ? Personal Lift Super Series Part Number DPL-35S, Designed to fit through a single door, AC or DC power options, Auxiliary platform lowering feature, Tie-down attachment points on each corner, Swivel-lock casters, Angled entry ladder, Ground and platform controls, Locking key switch, GFCI 110V electrical outlet in platform ? Options and Accessories: Tool tray, Fluorescent tube caddy, 110V or 220V at 50/60 hz, Operational warning light and descent alarm, Platform-locating laser light ? Standard Compliance: ANSI/SIA A92.3, CSA CAN3-B354.q-M82, CE indoor compliance, AS 1418.10-1996 ? THIS CLIN INCLUDES SHIPPING AND HANDLING COSTS TO GUAM (vi) N/A (vii) FOB Destination to Andersen AFB, Guam 96543 with a delivery of 30 days after receipt of contract award. (viii) FAR Clause 52.212-1, Instructions to Offerors ? Commercial applies to this acquisition. (ix) The evaluation criteria for award will be based off of the contractor?s ability to meet the delivery time, the ability to ship items FOB Destination, and price. (x) A completed copy of FAR provision 52.212-3, Offeror Representations and Certifications ? Commercial Items, must be submitted with the contractor?s offer. (xi) FAR Clause 52.212-4, Contract Terms and Conditions ? Commercial Items, applies to this acquisition. (xii) FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items applies to this acquisition. The additional clauses contained in 52.212-5 are also applicable: Clause 19 and Clause 24 (both of which are located within FAR Clause 52.212-5).
