Description
This is a combined synopsis/solicitation. The GLAC (Great Lakes Acquisition Center) intends to negotiate a firm fixed price contract to provide GENERAL INTERNIST OR FAMILY PRACTITIONER SERVICES in the Emergency Department at the VA Medical Center Milwaukee, 5000 West National Avenue, Milwaukee, WI 53295 (herein after called the VA or VAMC) for the base period of Date of Award through January 16, 2004. Request for Quotation #RFQ 69D-356-03 entitled "General Internist or Family Practitioner Services" is hereby issued for commercial items as full and open competition and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This announcement incorporates provisions and clauses in effect through FAR FAC 2001-15 and VAAR 97-10. NAICS Code 621111 applies for this procurement and the small business size standard is $8.5 million. This announcement/solicitation constitutes the only bid document that will be issued. A separate written solicitation document will not be issued. Contractor's proposals (cost and technical) are herein requested. A response to requirements listed below shall be submitted on contractor's letterhead or formal quotation form and must address all requirements listed in a separate technical proposal. Contractor's price and technical proposal must contain the name; title and signature of person authorized to submit proposals/bids on behalf of the contractor. VA Milwaukee has determined a requirement for ONE (1) Internal Medicine Physician or Family Practice Physician. The Physician is to work Monday through Friday 9:00 a.m. to 5:30 p.m. (total 40-50 hours per week) except in case of emergency and with possible rotation on evenings, nights, and/or weekends. The Physician must be a Board Certified/Board Eligible General Internist or Family Practitioner in Internal Medicine. VA will guarantee work starting from Date of Award through January 16, 2004, with the option to extend for up to two (2) three (3) month option periods. The VA reserves the right to terminate the contract or decrease the hours. The following is an non-all inclusive list of duties and responsibilities in a seven (7) bed, Level III, Emergency Department and the Urgent Care area of the Emergency Department: TREATMENT: (1) Function as a full team member of the interdisciplinary team serving civilian and military patients at the VA Hospital and Clinics (2) Evaluate and treat approximately 15-30 patients per 8-hour shift. Medical complexities vary and include Cardiovascular, GI, Pulmonary, Orthopedic, Surgical, Psychiatric, and Substance Abuse Detoxification (3) Provide history, physical examination, diagnosis, and initiation of medical treatment (4) Initiate and facilitate treatment and transfer in emergency situations (5) Evaluate patients with existing medical problems and complete appropriate diagnostic tests and medication adjustments (6) Referral of patients as needed to appropriate specialists, health care facilities, and coordination of home health services. Subspecialty consultation services are available 24-hours per day (7) Document assessments and plan of care for each patient in accordance with policies and procedures (8) Act as a consultant in clinical practice (9) Collaborate with other disciplines to ensure continuity of patient care (10) Collaborate and act as a patient advocate (11) Maintain knowledge of new and current clinical methods (12) Patient critical lab results may be communicated to physician via phone after hours by the lab (13) ACLS Certified required. SPECIAL REQUIREMENTS: (1) Respects confidentiality and follows established policy regarding patient records. (2) Observes VA policies and procedures relating to infection control, Fire and Safety, Information Security, Patient Safety (mandatory training). (3) Perform the required work in accordance with JCAHO standards. (4) Communicates and works cooperatively with other team members to provide comprehensive patient care. A full-time equivalent (FTE) is defined as a contractor's employee working eight (8) hours per day, five (5) days per week (40-50 hours per week). Contractor's employees must be present at VA and must be actually performing the required services for the period specified in the contract or the contract cost will be decreased accordingly during each billing cycle. A VA record-keeping system of contractor hours worked is established through time and attendance logs in the departments being served. The payments to contract personnel for any leave, including sick leave, holiday, or vacation time is the responsibility of the contractor. Leave granted to contract employees would be in accordance with contractor policies. VA is not obligated to the Contractor during Technician absence. QUALIFICATIONS: Personnel assigned by the contractor to perform the services covered by this contract shall: have a full and unrestricted license in a state, Territory, or Commonwealth of the United States or the District of Columbia, be subject to a background check, be proficient in written and spoken English, be an U.S. Citizen or permanent resident, and complete VA credentialing process for delineation of clinical privileges. The physician must be familiar with personal computers, have adequate typing skills to perform data entry (progress notes and physician orders), and be able to navigate through windows based computer programs. Evaluation of proposals offered will include the following factors: Professional qualifications, clinical skills/interpersonal skills, availability, past performance and price. Award will be made to the offeror determined to be the "best value" to the Government. Awarded offeror is hereby notified that the position would be considered "low risk" under the positions sensitivity designate in accordance with IL 90-01-6 entitled Contractor Personnel Security Requirements. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide services as listed above may respond to this solicitation by submitting cost and technical proposals (curriculum vitae) on their company letterhead or bid form.
