Description
Review, authorized physician medication orders, 7 days per week from 6:00PM to 8:00 AM Eastern time, Monday through Friday and 4:30pm to 8:00 am, Eastern Time, Saturday, Sunday & Holidays that are scanned, sent, and transmitted to the contractor. Contractor Pharmacists will enter via remote access, orders into the hospital medication order entry system. Support evaluation and management of suspected adverse drug reactions, adverse event and medication errors during after hours shift in the form of verbal and written drug information and biomedical reference materials. Incidents or Occurance reports to be completed by Hospital Staff. Provide reports to the hospital's pharmacy department as follows: 1) Daily shift "sign off" (faxed,voicemail or web based) to include summary of outstanding issues for follow-up by hospital pharmacy staff. 2) Monthly report on activities provided to hospital staff via after hours service. 3) Quarterly report on quality improvement indicators from vendor including: a. Potential prescribing errors and other medication errors. b. Suspected adverse drug reactions or events c. Drug information/educational needs of after hours hospital staff d. Drug distribution issues including storage, shortages and nonformulary drug requests. 4. Contact hospital's on-call pharmacist when the onsite service of a pharmacist is required. 5. Services provided shall comply with the applicable standards of the Joint Commission on Accreditation of Health Care Organizations including maintenance of competency training for all staff providing services to the hospital.
