- Description
- Issuance Date: January 13, 2003. Closing Date: January 31, 2003 at 3:00 pm. Honduran time. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is RFP-522-03-040 and is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-11. The USAID Honduras Mission requires to procure supplies to be used in medical centers. The contractor shall furnish the following supplies and quantities and deliver them to American Embassy, USAID-Honduras-Warehouse, Entrada Col. San Miguel, atras de Fabrica MASECA, Tegucigalpa, Honduras: 1-METAL SHELVES, Dimensions: 72"H, 48"L, and 12" W. With 6 shelves for each unit, Quantity: 412 ea; 2-FIBER GLASS CHAIRS OR HEAVY DUTY PLASTIC CHAIRS, Color light green or aquamarine. These chairs will be use in health centers. Quantity: 2,000 ea; 3-GENERAL TRANSFER STRETCHERS, Similar to GENDRON model No. 1025 chrome frame Stainless steel litter, dimensions 76?L X 23?W X 32?H. Heavy duty construction, 10? non conductive casters, With the following accessories: -1 pair swing slide rails, chrome, -1 conductive litter pad, -1 IV rod telescope and -1 pair of brakes. Quantity 20 ea; 4-OTOSCOPE/OPHTHALMOSCOPE SET, with fiber optic otoscope and halogen ophthalmoscope, to be used with battery. Adult especulas. Each set will have 10 halogen replacement bulbs. Similar or better than Welch Allyn models. This item is battery operated, no rechargeable Batteries. Will be used to operate this item and it is a desktop item. Quantity: 115 ea; 5-ADULT MANUAL RESUCITATOR, Complete with Lear PVC adult and child mask with soft cuff 22mm footing, with carrying pouch, with reservoir and valve. With caring case Quatity: 30 ea; 6-SINGLE SOAP DISPENSER, Heavy duty, foot operated with chrome stand, Half gallon polyethylene plastic jar, pump system, Stainless steel routable spout, 52? overall height. Quantity: 204 ea; 7-ADULT PHYSICIAN SCALE, Heavy base and lever system, white color, Kilograms and pounds scale. Measuring rod range 75cm to 195cm, Capacity: 350 pounds X 1/4oz and 160kg X 100g, Dual reading Model, Casters, Color white, Quantity: 115 ea; 8- BALANCE BEAM PEDIATRIC SCALE, Heavy duty, lever system. Must be in Kilograms and pounds. Desk type and mechanical with tray. Capacity 361lb X 1/8oz, Kg/g and pounds/oz routable bar. White color. Quantity: 120 ea; and; 9-DRI-CLAVE STERILIZER, Dri-heat type, chamber 18 ??W X 14 ??D, Temperature range 320-360 F, with two hours switch, thermometer, thermostat; with 3 large instrument trays, 115 volts/60Hz/650 Watts. Quantity: 70 ea; Please add shipping and handling, including in-land transportation and local custom broker services (in Honduras) costs in order to help the procurement to get through the Honduran customs upon arrival, and the grand Total Cost for a Firm Fixed Price, F.O.B. Offerors need to include a complete copy of the provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Provisions 52.212-4, Contract Terms and Conditions-Commercial Items and 52.212,5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
