- Description This synopsis for commercial supplies and services is prepared in accordance with the format in FAR Part 12.6 as supplemented with additional information included in this notice and Simplified Acquisition Procedures in Part 13. This notice constitutes the only solicitation; a Request for Quotation is being requested and written solicitation will not be issued. The NAICS code is 492110 and the size standard is 1500 employees. DESCRIPTION OF REQUIREMENT: The requirement is to furnish all labor, tools, parts, materials, facilities, equipment, vehicles, and transportation necessary for courier services. PERIOD OF PERFORMANCE: Basic Year plus four (4) option years. STATEMENT OF WORK: Courier Service for Wilford Hall Medical Center 1. SCOPE: Purpose is to establish a Blanket Purchase Agreement for courier service for all activities supported by WHMC. The 59th Medical Wing Fleet Management Office will provide the courier schedule. The Contractor shall furnish all labor, vehicles, and equipment necessary to accomplish the courier schedule provided. The courier route includes the locations and points of contact provided, but is not limited to scheduled runs. There will be, on occasions, ?hot shot? deliveries. The courier service will perform: pick up and deliveries to area hospitals and other facilities in the Greater San Antonio area. 2. LOCATION OF WORK: 59th Medical Wing Fleet Management Compound. Bldg 4957 will be the home base for the contract employee and the vehicle. 3. JOB VERIFICATION: 59th Medical Wing Fleet Management personnel will furnish the contractor with any special instructions for pick-ups or deliveries not on the daily scheduled run. 4. HANDLING: Contractor shall be responsible for damage/loss during transport and delivery of items. 5. HOURS OF WORK: Normal work hours are 0645 to 1700, Monday through Friday. 6. POC: Point of contact will be NCOIC, 59th Medical Wing Vehicle Management, located in bldg 4957. Points of contact for delivery other than Wilford Hall will be given to driver prior to departure run. The following provisions and clauses apply: 52.252-2, Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. The provision FAR 52.212-2, Evaluation-Commercial Items, is applicable to this solicitation and the blanks in section (a) are completed as follows: Price considerations. Offerors shall include with their quotes, a completed copy of 52.212-3, Offeror Representations and Certifications--Commercial Items. Far 52.212-4, Contract Terms and Conditions--Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercials Items, 52.222-41, Services Contract Act of 1965; 52.232-33, Payment by Electronic Funds Transfer--Central Contractor Registration, and DFARS 252.204-7004, Required Central Contractor Registration, are applicable to this acquisition.
