- Description: This is notice of intent to award Sole-Source in accordance with FAR 6.302-1, as supplemented with additional information included in this notice. No Solicitations or Request for Quotations (RFQ's) are being issued. The contract documents will included all incorporate all provisions and clauses in effect through Federal Acquisition Circular 2001-15. This procurement is Restricted to a Sole-Source. The NAICS is 423450 with business size standard 500 employees. A firm-fixed price contract is anticipated. The VA Puget Sound Healthcare System, 1660 South Columbian Way, Seattle, WA 98108 has a requirement to purchase: 16 voyager V420 Plus 4-way Ceiling Tract Lift Systems, approximately 30 2-Meter Kwiktraks, 30 3-meter kwiktraks, 16 90-degree curve kwiktraks, all of the Misc. Hardware items, and the required installation. All inquiries are to include a completed copy of the provisions at Far 52.212-3, Offer Representations and Certifications-Commercial Items, with quotations, (a copy of this provision may be requested by facsimile to 253-589-4140, attn: Mike Gibbs:. The following provisions and clauses apply to this action; 52-212-4 Contract Terms and Conditions Commercial items, 52-212-5 Contract Terms and conditions required to implement statutes and Executive orders-commercial items including clauses incorporated by reference under Section b (11-16)(19)21-22)(25). 52.212-1 Instructions to Offerors-commercial items, VAAR Clauses 852.211-77 Brand Name or Equal, 852.211-70 Requirements for Operating and Maintenance Manuals, 852.211-71 Guarantee Clause and 852.270-4 Commercial Advertising.
