- Description
- Establishment of a Capitated Community Based Outpatient Clinic (CBOC) in Henderson, NV. Contractor shall provide Primary Care services at their facility, to beneficiaries of the Southern Nevada Healthcare System, Las Vegas that fulfill the VA's healthcare delivery requirements, in accordance with all terms, conditions, and provisions of the solicitation. The contractor shall provide for the continuous delivery and management of primary care for all assigned VA patients. Veterans receiving services under this contract reside primarily in the catchments area of Henderson, NV. The contractor will be expected to subcontract for any required service that it does not provide, at no additional cost to the Government. It is the VA's intention to solicit proposals on a competitive basis and award a contract that provides for an "All Inclusive" capitated rate (per member, per month) method of payment. The facility physical location must be within Henderson, NV. The Government prefers that the Contractor secure the current CBOC facility located at 2920 North Green Valley Parkway, Suite 215, Henderson, NV 89014. If not, the Contractor's facility must be physically located within a five (5) mile radius of the current CBOC facility and within the city limits of Henderson, NV. The contract includes a Base Year (one year from date of award) plus 4 one-year renewal options that may be exercised at the option of the Government. The total duration of the contract will not exceed 5 years. This solicitation is issued as commercial item procurement in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. The NAICS is 621111 and the size standard is $8.5 Million. This is an unrestricted solicitation and is advertised as full and open competition. The solicitation will be issued o/a September 30, 2003. No telephone request will be honored. Closing date for all requests is September 19, 2003 at 4:00 P.M., Pacific Standard Time (PST). VA will not be responsible for any cost associated with your expression of interest (e.g. proposal (s) & solicitation (s), etc.). Offerors are advised that they are responsible for obtaining any amendments. All questions and/or Requests for Information regarding the solicitation must be in writing on company letterhead addressed to the Contracting Officer.
