Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 506-19-03 is being issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-15. NAICS Code 423450, 500 employees small business size standard applies. Items will be purchased and delivered to the VA Ann Arbor Healthcare System, Surgery Service, 2215 Fuller Road, Ann Arbor, MI 48105. Prices shall be submitted for the following: One Neurosurgery Stealthstation Imaging System. Features needed on the system: Using a unix-based computer system; ability to separate the camera cart and viewing cart; ability to intra-operatively re-register the patient; provide both active and passive technology as it pertains to instruments and reference frames; ability to integrate secondary viewing screens into the system, as well as virtual placement 3D images of screws; voice and photo prompts in the software; laser contour registration for non-fiducial based cranial/ENT procedures; software which allows surgeon to predict and control the placement of the zone of accuracy to focus on specific anatomical regions; allow for image settings and planning fuctions can be adjusted at any time in the OR; reference frames can be oriented in multiple positions giving the user a greater degree of flexibility in equipment setup; system must be able to adapt to both ENT and Spinal/Cranial proecures. The ENT components must: allow user to choose between on-site or off-site scanner facilities; require no headset for the patient, using anatomic landmarks for registration; have built-in modem for immediate diagnostics and software upgrades; use proprietary, multi-factor calculation that depicts actual clinical accuracy for consistent, sub-millimetric surgical precision; allow no interference of the optical system from other OR instruments/equipment; allow user-defined views such as translation sliders, magnification scale with multiple options, multiple viewing windows per screen, and 3D cut-away views. Warranty period of one year is required. Price is to include shipping, set up installation, training, software, hardware, warranty and support services necessary to obtain full operational value. The following provisions and clauses apply to this solicitation: 52.212-1 Instructions to Offeror-Commercial Items, 52.212-4 Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Order Commercial Items with (b) (1),(14),(15),(16),(17),(18),(19),(21),(24), and (30) checked. FAR 52.233-2 Service of Protest, VAAR 852.233-70 Protest Content, VAAR 852.233-71 Alternative Protest Procedure and VAAR 852.270-4 Commerical Advertising also apply to this solicitation. Quotes will be evaluated on the following criteria: technical capability of equipment, price and service agreement offered. Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a completed copy of the provisions 52.212-3 Offeror Representations and Certifications-Commerical Items, literature/brochures on proposed equipment and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on ther internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/. Quotations and all required information must be received by 3:30 PM September 12, 2003. Quotations shall be submitted to Brenda M. Johnson, Contracting Officer, VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. Faxed quotations are acceptable and may be faxed to (734) 213-3833. Solicitation number 506-19-03 must be listed on all documents for identification purposes. Only offers prepared in accordance with this notice will be considered.
