- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. This solicitation is a small business set-aside in accordance with Section 15 of the Small Business Act; the National American Industry Classification System Code is 423450 and the Small Busine ss size is 100 employees. The Winn Army Community Hospital, Fort Stewart, GA has a requirement for 2 Audiometric Booth, MLD 120M-8, double wall eight man booths furnished, delivered, installed. Complete with lights, jack panels headphone hooks, divider c urtains, test subject stools, acoustic door with view window shock mount flooring ventilation connection for supply and return. Must include 2 copies of service maintenance and operator manuals. For additional specifications contact Carolyn Lovett at (706) 787-7960 and a faxed copy will be provided. The booths will be purchased in pairs as they will be installed at two different facilities at Fort Stewart on September 15, 2003. Proposals are due on August 18, 2003 by 3:00 PM Eastern Standard Time. Award w ill be made based upon the best value to the government. The Government intends to award a firm fixed price contract. Item 0001 Audiometric Booth, 2 each, Unit Price $ _______; Total Price ______. Delivery/Site Location: Delivery shall be FOB destinatio n for all items to the delivery address as stated above. The provision at 52.212-1, Instructions to Offerors - Commercial Items, is hereby incorporated by reference. All offerors must include a completed copy of the provision at 52.212-3, Offeror Represe ntations and Certifications - Commercial Items with their proposal. The clause at 52-214-4, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition; additional clauses cited in 52.21 2-5 that apply to this acquisition are: 5,13,14,15,16,17,18,21,24,27,29.
