- Description
- The Department of Veterans Affairs, VAMC San Juan PR, is seeking for contractor(s) to establish a consignment agreement to furnish soft good products listed in the solicitation package - schedule B for Prosthetic Service located at the VA Medical Center, 10 Casia Street, San Juan, Puerto Rico, 00921-3201. The consignment agreement included but not limited to furnish and maintain an inventory of soft good products. The period of performance will be for an initial base period of six (6) months from September 8, 2003 or date of award if later through March 31, 2003 with the option to extend the services for an additional period not to exceed six (6) months. This procurement is been issued 100% Set aside for Small Business. The North American Industry Classification System (NAICS) code is 339113 and the business size standard is 500 employees is applicable to this acquisition. THE SOLICITATION WILL BE MADE AVAILABLE ON OR ABOUT AUGUST 15, 2003 with a due date of August 29, 2003, by 10:30am PR Local time. Copies of the solicitation may be obtained at no charge at the Business Opportunities System website http://www.bos.oamm.va.gov. This solicitation will be available in electronic format ONLY. The solicitation number for this acquisition will be 672-372-03. This solicitation is a request for proposal (RFP). Dissemination is limited to electronic medium and the VA WILL NOT PROVIDE HARD COPIES. Any amendments issued to this solicitation will also be available at the URL address. Once the solicitation is posted Offerors are advise that they are responsible to review and for monitoring this solicitation (672-372-03) in the website frequently to ensure that they have most up to date information about this acquisition. Offeror are responsible for downloading their own copy of the solicitation and amendments. Questions shall be submitted IN WRITING by or before August 19, 2003 @ 3:30pm (PR Local Time) at the following e-mail address maribel.santadelapaz@med.va.gov or via facsimile at (787) 641-8364.
