- Description
- Contractor shall provide all necessary labor, supervision, equipment, and materials to furnish and install concrete grave liners at Mountain Home National Cemetery, P.O. Box 8 *VAMC Bldg. 117*, Mountain Home, TN 37684. Contract covers base period October 1, 2003 (or date of contract award, whichever is later), through September 20, 2004, with up to four 1-year options to extend the term of the contract through September 30, 2008. This acquisition is 100 percent set-aside for small business concerns. North American Industrial Classification System (NAICS) Code 327390 and a size standard of 500 employees are applicable. All responsible small business sources may submit a quote. Request for a hard copy of the solicitation must be in writing to Mrs. Janice E. Corlew, Contract Specialist, and accompanied by a check or money order in the amount of Five Dollars ($5.00) (non-refundable)payable to "U.S. Department of Veterans Affairs." Please reference Solicitation Number IFB 786-36-04 on remittances. No telephone requests will be accepted. Hard copy solicitations will be isued via the ordinary mails on or about August 29, 2003. The solicitation may also be obtained FREE OF CHARGE, from VA's Business Opportunities System via the internet at the following URL address on or about August 29, 2003: http://www.bos.oamm.va.gov/solicitation?number=786-36-04. Any amendments to the solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments. See Commerce Business Daily Numbered Note 1.
