- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. Solicitation R1-04-03-93 is issued as a Request for Quote (RFQ). The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-15. The clauses and provisions referenced in this solicitation can be found in full text form at http://www.arnet.gov/far. This is a total small business set-aside and the small business size standard is 500 employees. The North American Industry Classification Code is 332311 Prefabricated Metal Building and Component Manufacturing. One contract will be awarded and will be firm-fixed-price. A quotation is requested for Item No. 1, Supply and Deliver of a HazMat/ Herbicide Storage Facility, with the following specifications: Dimensions, no bigger than 12 foot by 7 foot by 9 foot height; Roof load 115 psf; 3-hour fire rating; standard passive ventilation; maintain interior temperature between 40 ?90 degrees Fahrenheit, 5-pound ABC Extinguisher; 1 Interior Light (wiring & equipment must be Class I Division I; Item No. 2, Optional Shelving with lip, prefer one 12 foot shelf and one 7 foot shelf. Please submit options with pricing. Item No. 3, Self-contained Safety Eyewash Station. Propose delivery date and off-loading capability, (Forest Service will complete final electrical hook-up), FOB Destination to St Joe Ranger District, Avery Office located at Hoyt Flats (from St Maries, east on Forest Highway 50 approximately 47 miles to Hoyt Flats). Delivery shall be made on regular working days (Monday through Friday) during normal business hours (7:30 a.m. to 4:00 p.m.) excluding Federal Government Holidays. Contractor shall notify Suzanne DiGiacomo at the St Joe Ranger District (208) 245-6008, 2 weeks prior to delivery. The contract will be offered to the best value offer made to the Government, considering suitability of product and price. The provision at FAR 52.212-1 - Instructions to Offerors-Commercial Items applies to this acquisition. Offerors are required to complete and include a copy of the following provisions FAR 52.212-3, Offer Representation and Certifications-Commercial Items with their quotation. The FAR Clauses 52.212-4, Contract Terms and Conditions Commercial Items, and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Order Commercial Items apply to this acquisition.
