Description
(i) This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted) on RFQ reference number 111111TESTEXAM. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14. (iv) This procurement is being issued as a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 54199 with a small business size standard of $5.0 Million. (v) This combined synopsis and solicitation is for the following commercial services: CLIN 0001- Test Examiner/ test supervisor (vi) DESCRIPTION OF AGREEMENT: The contractor shall furnish the following services according to the TEST EXAMINER STATEMENT OF WORK. A. Description of Work: The Test Examiner shall furnish a minimum service for one unit, which will be the conduct of testing services and other duties in the Service Delivery Summary listed below. The Testing Control Officer/ Alternate Testing Officer/ Interim Testing Control Officer (TCO/ATCO/ITCO) will determine when services under this contract will be required and will make calls for the services. ?Calls for services? is defined as the notification to the contractor that an appropriate number of tests are scheduled for a future date. Such notification must be made at least 24 hours in advance. Calls will be informal and may be in writing, by telephone, or other means of available communication. A list of the names of officials authorized to make calls for services will be provided to the Contractor. A standing call for services is hereby established as Monday-Friday excluding legal holidays. The TCO/ATCO/ITCO will provide at least 24 hours notice to the contractor if services are not required on a particular day. Except in the case of unexpected illness or other emergency, the contractor will provide 24 hours notice of inabilty to provide service. Such instances are expected not to exceed 3 absences during the course of the contract. Individual will not be paid for referenced absences. Contractor will be provided access to the base (if required), and the Government will provide al necessary equipment to conduct the work. Government provided equipment includes, but is not limited to, furnished office space, computer hardware and software to fullfill duties, office supplies and equipment. Shall not subcontract any portion of this Statement of Work to another individual. B. SERVICE DELIVERY SUMMARY: (a) Perform the duties and responsibilities applicable to test examination procedures as specified in the DANTES Examination Program Handbook and those duties and responsibilities for all other test administration germane to the Testing Program. Adhering to testing instructions and test security is paramount. (b) Provide services, perform duties, and be responsible for administering all other testing provided by the Testing Section in accordance with the policies applicable to each of those tests. Such tests may include make-up tests constructed by faculty, of the various contracted institutions. (c) Process test receipts in Air Force Automated Education Management System (AFAEMS) and notify corresponding examinees for special order tests, Air Force Institute of Advanced Learning (AFIADL) tests, and college tests within 1 duty day of receipt. Explain the need for expeditious testing and return of special order tests. (d) Return answer sheets/tests to the appropriate scoring agency within prevailing time constraints per the shipping/transmitting requirements. Packaging and shipment procedures shall be in accordance with the respective agency/TCO/ATCO/ITCO. (e) ensure accurate processing of test results within one duty day of receipt; File one copy in examinee's education file, one copy in Education Center?s file, and mail one copy to examinee. (f) Inventory all other tests daily, weekly, and monthly. Destroy all other tests quarterly or as required by the appropriate agency/TCO/ATCO/ITCO. (g) Notify the TCO/ATCO/ITCO if the testing room(s) does not have adequate lighting, heated, quiet, or if any other deficiency exists that would make testing difficult. Ensure all lights are turned off, electrical appliances are turned off, and doors and windows are closed and locked at the end of scheduled testing periods. Ensure testing desks and/or tables and chairs are arranged neatly and that any litter produced during testing is picked up prior to departing the center. Be responsible for all keys issued to him/her and shall pay for replacement of keys and/or installation of locks if the occurrence of lost keys presents itself. Report any damage to equipment used in the Testing Rooms to the TCO/ATCO/ITCO immediately. (h) Assist the TCO/ATCO, ITCO in preparation for and participation in official inspections and Staff Assistance Visits. (h) Fully comply at all times with those items listed on Form 2158, DANTES Testing Section Annual Inspection Report. (i) The Contractor will provide typing as required and/or computer input as appropriate. (j) Shall comply with the installation's safety and security requirements applicable to the Testing Section facilities and equipment. (k) Contractor will submit invoices (DD FORM 1572) on a monthly basis for the portion of the services rendered for the period. A unit of service performed is defined as the Test Examiner?s administering a minimum of five scheduled tests., or as determined by the TOC/ATCO/ITCO.. If, due to no-shows the minimum of five tests scheduled does not result' in five tests actually administered, the responding contractor will be paid as if five tests were administered. The contractor will test walk-ins or other unscheduled examinees, but one unit of service will normally not contain more than 15 tests. No-shows will be annotated as such on the invoice(s). Delivery of service will be measured by the attached standards. Initial evaluation of delivery of service will be three months from contract commencement. Follow-up evaluations will be every six months. C. JOB QUALIFICATIONS: (a) As stated in the DANTES Examination Program Handbook. Contractor must meet education requirements of the Office of Personnel Management Handbook X-1118 for Education Specialists. (c) Computer application skills are desirable. (d) A personal security invesitigation may be required. An unsatisfactory security investigation may be grounds for immediate termination of the agreement. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation. (xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation.
