Description
The Centers for Medicare and Medicaid Services intends to award, under Simplified Acquisition Procedures, on a sole source basis a firm fixed price award to Summit Health Institute for Research and Education, Inc. (SHIRE), 440 First Street N.W., Suite 440 Washington, DC 20001. The project is entitled ?Intergenerational Faith-based Diabetes Prevention and Self Management Education Program for Use in the African American Community?. The purpose of the project is to contribute to the elimination of health disparities with regard to diabetes prevalence, complications, and medical treatment experienced by African American Medicare beneficiaries in Montgomery County, Maryland. The goals are to help diabetics, who are enrolled in Medicare, attain improved health status. Second goal is to seek the institutionalization of this intergenerational pilot project in faith-based community-based organization serving Medicare enrollees in the county. The following objectives are envisioned: 1) Ensure the availability of an informed youth corps to assist Medicare beneficiaries in effectively detecting, managing and controlling diabetes. 2) Establish, maintain and support one-on-one health-promoting relationships between participating youth and seniors. 3) Engage and work cooperatively with two faith-based organizations and their health ministries in developing, conducting and evaluating the project. 4) Provide a ?how-to? monograph, which describes the lessons learned and outcomes. 5) Sponsor countywide health forum involving youth and seniors about the importance of diabetes management, with on-site screening and a 40 percent follow-up 3-months and 6-months after the health forum. 6) Provide pre and posttests. SHIRE has established a sterling reputation for engaging communities for the delivery of health promotion and disease prevention services. SHIRE projects implemented with HHS support include: the organization of Diabetes Action Teams (DATs) in five states focused on community-wide approach for use by DATs in assessing and documenting their progress and outcomes; and development of HIV/AIDS materials on rapid testing targeted to pregnant at-risk Latino/Hispanic and African American women and their health providers. These materials have been distributed and technical assistance has been provided in Baltimore City and Prince George?s county, Maryland, as well as in the District of Columbia and U.S. Virgin Islands. SHIRE?s work in developing community partnerships has also been extensive. In Howard County, Maryland, SHIRE has been responsible for planning, with the support of a private foundation to aid children and youth in choosing health-related careers. SHIRE has also convened statewide forum on diabetes, cardiovascular disease and depression, with a focus on strategies for prevention and early detection. SHIRE has also engaged in community consultation and strategic planning that has resulted in the formation of countywide multicultural health promotion campaigns in Texas and Maryland. In Montgomery County, SHIRE has played an instrumental role in implementing an innovative project in Montgomery County, Maryland that has helped synergize community resources. Through a public/private partnership, a Youth Health Builders Corps (YHC) has been formed, whose members, ages 12 to 18, are being trained as diabetes educators and advocates. The youth have been recruited from two faith institutions in the county, and have already launched activities, including a website they have designed, which are intended to inform other youth, congregants and community members about diabetes ? the nature of the disease, risk factors, as well as effective prevention, management and control strategies. This project will serve as the springboard for the proposed initiative.
