- The Social Security Administration (SSA) is seeking sources to provide all management, supervision, labor, materials, tools and equipment to remove existing sand and wood chips from the playground area and furnish and install playground quality sand and wood chips at the Harold Washington Social Security Center (HWSSC), 600 West Madison Street, Chicago, Illinois 60661. Contractor employee(s) performing on site at SSA facilities must meet the agency's security clearance. They must complete some or all of the listed forms. GSA 176, Statement of Personal History, FD-258, Fingerprint Chart, The Fair Credit Reporting Act(FCRA) Authorization Form, For Non-US Citizen, legible photocopy of the Work Authorization Permit and Social Security Card.
