Description
NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. The Georgia Army National Guard is seeking Sources to provide two-step bidding for Point of Service Laboratory Equipment to conduct the following tests: Hemoglobin or Hematocrit, Total Cholesterol, PSA, fasting Blood Sugar, Lipid Profile (Cholesterol, LDL , HDL, Triglycerides), Urine Glucose and Albumin, Urine Specific Gravity and Micro-urinalysis, and Drug/Alcohol Panel. All requirements shall be in compliance with Army Regulation 40-501. This constitutes Step I where Sources will be provided and opportu nity to submit technical proposals. Technical proposals shall include but are not limited to equipment literature, specifications, standards, operational directions, supply requirements, space and power requirements, warranty technical approach for support ing required tests and past performance of equipment (consisting of a minimum of 5 years manufacturing and usage history). Acceptable technical proposal shall be invited to demonstrate acceptable items. Demonstrations & Testing determined to be acceptable will be invited to submit price quote. Technical proposals are due 14 August 2003, demonstrations shall be conducted approximately 2-5 September, and price quotes shall be due approximately 15 September 2003. Award shall be firm-fixed price and may be made to multiple vendors. Multiple technical proposals may be submitted by each offeror for different technical approaches. No phone calls shall be accepted with regard to this solicitation. This solicitation shall not be considered for Hubzone set-aside. GSA FSS vendors are prefer red. This acquisition is 100 percent set-aside for small business concerns. North American Industry Classification System (NAICS) 523450 and a size standard of 100. See Note 1.
