Description
The 377 CONS/LGCA, Kirtland AFB, New Mexico, intends to award a sole source contract in accordance with FAR 6.302-1(a)(1), to Anderson, Niebuher & Associates, Northpark Corporate Center, 6 Pine Tree Drive, Arden Hills, MN 55112-3747 (651/486-8712) for a base and four one-year options in order to provide on-site training workshop on Questionnaire Design and Reporting. CLIN 0001: Conduct Annual On-Site Training - Base Period. Period of Performance: 11 Aug 2003 - 10 Aug 2004. Contractor shall conduct an annual one-day training workshop on questionnaire design and reporting on-site at Kirtland Air Force Base, NM in accordance with (IAW) the attached Statement of Work (SOW). Annual training shall include two (2) accredited instructors, presentation materials, workbooks and class handouts to accommodate a maximum of 25 participants. SUBCLIN 0001AA: 1 Session. Annual training shall be conducted one time per year on-site at Kirtland AFB, New Mexico IAW the attached statement of work. Annual training shall be scheduled no less than two weeks in advance by 377 CONS/LGCA in coordination with AFIA and the service provider (contractor); SUBCLIN 0001AB: 1 Lot. Instructor Travel. Travel to include airfare in commercial class; lodging for not more than two nights, transportation to and from airport and local (new Mexico) travel in a rental car at a rate no higher than a compact vehicle associated with this training; and meals for two instructors. Cost shall not exceed the allowable lodging and meal per diem currently in effect for government employees IAW the Joint Federal Travel Regulation (JFTR). The invoice for this contract line item number (CLIN) shall be accompanied by receipts validating the exact cost of airfare, transportation, lodging, and meals for each instructor. STATEMENT OF WORK For QUESTIONNAIRE DESIGN & REPORTING TRAINING 1. Contractor shall provide on-site training workshop on questionnaire design and reporting with two accredited instructors, presentation materials, workbooks and class handouts to accommodate a maximum of 25 participants. The contractor shall provide the Government with class attendance and instructor comment sheets for each student attending the class. 2. Objectives of this class are to train inspectors on designing questionnaires for obtaining maximum information in a specific format. Inspectors will be trained in the formulation of survey questions to be used in Air Force wide surveys, and interpretation of data collected. 3. The topics covered shall include, Questionnaire Design, Question Formulation and Data Collection Techniques. Course will be taught at Kirtland Air Force Base Headquarters-Air Force Inspection Agency (AFIA), AFIA University Classroom, 9700 G Avenue SE. 4. The Government shall provide class-meeting facilities to accommodate 25 participants and audio-visual equipment to include overhead projector/screen, LCD projector, microphones and extension cords. 5. The course shall be conducted on 26 August 2003. The schedule for the course will be held for a one-day period from 9 am to 4 pm (MST), with one-hour scheduled for lunch, for a total of 6 hours of instruction.
