- The BIA, San Felipe Elementary School, San Felipe Pueblo, New Mexico has a requirement to provide occupational therapy and psychological services during the period of August 18, 2003 through July 31, 2004, with four (4)option year periods. Occupational Therapy Services will be provided for a total of 113 days and Psychological Services will be provided for 38 full days and 38 half days. The contractor will provide a certified psychologist (s) and occupational therapist, supplies, materials, equipment and testing materials to perform the work. Quotes are being requested to provide both Occupational Therapy Services and Psychological Services; or, to provide only Occupational Therapy Services or Psychological Services. Therefore, a single award or multiple awards may be made based upon the best value to the Government. Requests for the solicitation, which will be released on or about July 23, 2003 may be faxed to (505) 248-6953 or emailed to dgutierrez@bia.edu. No telephone requests will be accepted. All responsible small business sources may submit a quote, which will be considered. This requirement is being solicited as a commercial item in accordance with FAR Part 12, Acquisition of Commercial Items; and, as a Request for Quotes in accordance with FAR Part 13, Simplified Acquisition Procedures. All correspondence should include the solicitation number, your full name company name, address and phone and fax numbers. Requests or questions by telephone are strongly discouraged. Faxed quotes will be accepted.
