Description
This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The solicitation number is RFQ is 246-03-00394. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-13. The NAICS is 421450 and the small business size is 500 employees. Offeror=92s proposals are hereby being requested in response to the following requirements under a firm-fixed price contract for two injection systems, one each to be delivered to the Hunter Holmes McGuire Department of Veterans Affairs Medical Center, Richmond, Virginia and the Department of Veterans Affairs Medical Center, Hampton, Virginia. Offers shall be submitted on offeror=92s letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. The injection systems must meet the following minimum requirements: (1) system must provide hold and pause programming to allow for the adjustment of protocol volume and flow rate during the pause phase (2) the Keep Vein Open (KVO) must be able to function independently from the injection profile (3) the syringe must be at least 115ml to allow for KVO and multiple injections (4) the injector head must provide a mechanism that allows the user to confirm that air has been expelled from the fluid path (5)there must be a minimum of 6 programmable phases to tailor the injection protocol (6) other features are to include inject delay, a stopwatch, scan delay (7) state of the art fiber optics between the scan room and the control (8) an injector head that can be positioned on either side of the patient=92s table. Quotes for the systems are to include services manuals and power cords. Installation of the systems are the responsibility of the contractor and charges are to be annotated as a separate line item. Quotes shall be valid for 30 days from the offer due date. All responses must include the following: (1) descriptive literature and product number for the item offered proving the product offered meets the above requirements (2) delivery time after receipt of order (3) warranty information (4) cost. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2 Service of Protest, 852.233-70 Protest Content, 852-233-71 Alternative Protest Procedure.=20 Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1, a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items, and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov and www.va.gov/oa&mm/vaar/ or fax a request to 757-728-3132. Quotations and all required information must be received by 4:00pm local time August 6, 2003 Quotations shall be submitted to Barbara Simmons, Contracting Officer (90C), VA Medical Center, Hampton, Va. 23667. No faxed offers will be accepted Solicitation number RFQ 246-03-00394 shall be listed on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered.
