(CMI) 560 E. 34th Avenue Anchorage, Alaska 99503-4196 PROPOSALS ARE DUE NO LATER THAN: No Later Than 4:00 p.m. (7/24/03) SUBMIT TO: Alaska Job Corps Center Attn: Jeremy Cotton, Purchasing Agent 800 E. Lynn Martin Drive Palmer, Alaska 99645 CONTRACT TYPE: Fixed Price, Services EVALUATION CRITERIA: - Cost of Services - Qualifications of offerer with particular emphasis on experience with target population - Range of support provided, including consideration of support facilities personnel and flexibility in meeting requirements of the Center. CONTRACT DOCUMENTS: The attached Proposal Package contains the Statement of Work, Conditions of the Contract and Specifications describing the extent and requirements of the work. PERIOD OF PERFORMANCE: BASE YEARS - 01 August 2003 through 31 July 2004 OPTION YEAR 1 - 01 August 2004 through 31 July 2005 OPTION YEAR 2 - 01 August 2005 through 31 July 2006 OPTION YEAR 3 - 01 August 2006 through 31 July 2007 AWARD DATE: ON OR ABOUT – 29 July 2003 The owner reserves the right to reject any and all proposals. INSTRUCTION TO OFFERER PROPOSAL Make proposal in accordance with the following instructions: 1. Submit offer on attached Proposal Form. Fill out all blanks, make no additions, erasures or alterations to the Proposal Form. An authorized official of the company making the offer must sign proposal Form. 2. Each bidder shall carefully examine the contract documents and shall review the existing Standing Orders, Job Corps Federal Regulations, and the Job Corps Policy and Requirements Handbook (PRH), prior to submitting a proposal, to familiarize himself with all conditions under which the services are to be performed. 3. Deliver proposal to Jeremy Cotton, Purchasing Agent, Administration Building, Alaska Job Corps Center, 800 E. Lynn Martin Drive, Palmer, AK 99645 at or before the date/time stated in the Request For Proposal in a sealed opaque envelope marked "PROPOSAL FOR PHYSICIAN CARE" and name of the bidder. Proposals delivered via Fed Ex, Express Mail, UPS, etc. are acceptable. Proposals received after the time stated in the Request For Proposal will be returned unopened. INTERPRETATION OF CONTRACT DOCUMENTS Should a bidder find discrepancies in, or omissions from, the proposal package, or should he be in doubt as to their meaning, he shall contact the Purchasing Agent, Jeremy Cotton, (907)746-8803, for an interpretation thereof prior to five working days of the time of proposal due date. The interpretation of the documents will be made by written Addendum; a copy of which will be issued to each bidder having received a Request for Proposal. ADDENDA OR BULLETINS Any Addenda issued prior to the proposal due date become part of the contract documents; shall be covered in the proposal; and become a part of the contract. AWARDS OR REJECTION OF BIDS The contract, if awarded, will be to the most qualified lowest responsive responsible bidder. The owner, however, shall be the sole judge as to who is responsive and responsible and reserves the right to reject any or all bids and to waive any irregularity or technicality in proposals received. BIDDER QUALIFICATIONS All personnel providing services must be licensed to provide medical services in the State of Alaska. PATIENT POPULATION Services are to be provided to all students attending the Center. The Center is designed to have 250 students with ages ranging from approximately 16 to 26 years old. It is expected that the majority of students will be in the 16 to 21 year range with an approximately even numbers of male and females. DOCUMENTS TO BE FURNISHED WITH ALL PROPOSALS ¨ Proof of insurance availability in amounts specified in Section 11, paragraph D. q Copy of current State of Alaska Mental Health Professional license of all personnel that will be providing services. q Curriculum vitae for each professional providing services, to include continuing education or professional development courses taken in the past two (2) years. PROPOSAL FOR NON-PERSONAL SERVICE PHYSICIAN CARE Date: OFFER OF doing business under the laws of the State of Alaska as: ¨ a corporation ¨ a partnership consisting of ¨ an individual trading as To: Chugach McKinley, Inc. dba Alaska Job Corps Center 800 E. Lynn Martin Palmer, Alaska 99645 The undersigned agrees to provide students of the Alaska Job Corps Center, referred to henceforth as the Center, with the services as stated in the Statement of Work. Such services will be provided for an estimated average of eight (8) hours per week for 50 weeks per contract year. The services will be provided at the facilities located at the Center. Remuneration for services rendered on Center will be at the rate of: BASE YEARS: Period of performance 01 August 2003 through 31 July 2004 Rate of $ per hour x 8 hours per week TOTAL: OPTION YEAR 1: Period of performance 01 August 2004 through 31 July 2005 Rate of $ per hour x 8 hours per week TOTAL: OPTION YEAR 2: Period of performance 01 August 2005 through 31 July 2006 Rate of $ per hour x 8 hours per week TOTAL: OPTION YEAR 3: Period of performance 01 August 2006 through 31 July 2007 Rate of $ per hour x 8 hours per week TOTAL: Witness Respectfully submitted, (Seal if Corporation) By Title Taxpayer Identification No (TIN) Business Address STATEMENT OF WORK PHYSICIAN CARE ALASKA JOB CORPS CENTER SECTION I. GENERAL OUTLINE Services to be provided will include, but not limited to, those services specified in the Job Corps Federal Regulations (Title 20 Code of Federal Regulations, Part 638.510) and in the Job Corps Policy and Requirements Handbook (PRH), Chapter 6. Services include: 1. Direct medical services to students. 2. Assist in the development, implementation, and monitoring of the total Center Health Program. The extent of the work is further described in Section III, Extended Statement of Work SECTION III EXTENDED STATEMENT OF WORK A. The services provided shall include but not be limited to those Medical Physician Health services enumerated in the Job Corps Policy and Requirements Handbook (PRH), Chapter 6 and the Physician technical Assistance Guide (TAG) 6-K. These services include: 1. The provisions of direst medical service to Alaska Job Corps Center students, shall be as follows: (a) Perform physical examinations on all students within two (2) weeks of their arrival on Center, which may include gynecological exams on female students. (b) Diagnosis and treatment of illness and/or injury of students. (c) Advise Health Services staff on preventative immunizations, skin tests, and chest x-rays for tuberculosis and control of communicable disease within CMI and Job Corps guidelines. (d) All necessary referrals for specialist and institutional care, with authorization by Health Services Manager. (e) Supervision of care for hospitalized students, where possible. (f) On-call services at all time, except where prior arrangements have been made and the Center Health staff have been duly informed. Were feasible, the Center Physician will use his/her private office for evening, weekend, and holiday treatment of students to avoid excessive use of the hospital and/or emergency room. (g) Physician may delegate no more than (3) of the required physician hours to a nurse practitioner/PA to provide routine medical services within the licensee’s scope of practice. 2. Assist in the monitoring of the Center Health Program, including: (a) Advice to the Center Director and Health Services staff in all medical or health related matters. This shall include the advisability of medical separations, cost of services, and any other health areas deemed appropriate by the Center Director. (b) Oversee a system, which promotes the coordination and integration of Health components (medical, dental, mental health, environmental health, and health education) with other Center activities. This should be done in cooperation with the Health Services Manager, Center Dentist, and Mental Health Consultant. (c) Consult with center staff on ordering, accounting for, and safeguarding health equipment, supplies and medications. (d) Provide input as requested on the implementation of the Health Education Program. (e) Advise on the maintenance, confidentiality, and safeguarding of all health records, in congruence with center, state, and federal regulations; comply with center’s HIPPA procedures for protection of student’s health information.. (f) Reviews, approves and maintains Standing Orders for medical care in accordance with Technical Assistance Guide 6-Q. 3. The securing of formal agreements for all necessary support services. (a) Twenty-four (24) hour emergency coverage for such times as the Center Physician is not available. (b) Access to necessary inpatient care for psychiatric care, the Center Physician may transfer this responsibility to the Center Mental Health Consultant. Formal agreements with hospitals should be made with a copy of each current agreement maintained in the Health Services Manager’s office. (c) Access to necessary specialist care. All referrals must be pre-approved by the Health Services Manager. (d) Assist in the arrangements for the purchasing of most appropriate and lowest cost medications, supplies, x-ray and laboratory services, utilizing equipment and supplies in place at the Alaska Job Corps Center. 4. The Center Physician agrees to adhere to the policies, procedures, and guidelines established by Job Corps. The Center Physician with the concurrence of CMI and the Center Director, may designate any of his/her responsibilities set forth in the agreement to appropriate qualified staff. 5. The Center Physician will have primary responsibility to implement the Health program and may be assisted by the Job Corps National Health Office staff and/or Regional Health Consultants, who will act under guidance and direction from the National Nursing Consultant, Job Corps, Washington D.C. 6. Center Physician aggress to indemnify and save CMI harmless from any expenses, including attorney’s fees and also claims on account of damage to property or bodily injury (including death) which may be sustained by Center Physician or his/her employees in connection with work performed. The Center Physician agrees to provide necessary liability insurance for him/herself and his/her employees at no additional cost to this agreement. 7. The Center Physician agrees to make no public statements concerning Job Corps students or Job Corps activities without written approval of the Center Director or hi/her designated representative. 8. All research projects and/or publications relating to Job Corps students or health activities will be fist cleared by the Job Corps National Health office, Job Corps, Washington D.C. through the Center Director. 9. Center Physician is to keep written records at the Center, reasonable detail of work performed under this agreement, and upon expiration of the contract, or as soon thereafter as possible, deliver to CMI. All such written records and any other data and written information prepared during the period of the contract, resulting from the services to CMI hereunder or furnished by CMI. CMI’s medical offices shall have the right to examine all records or reports pertaining to this contract at any time.
