Lynn Martin Drive Palmer, Alaska 99645 CONTRACT TYPE: Fixed Price, Services EVALUATION CRITERIA: - Cost of Services - Qualifications of offerer with particular emphasis on experience with target population - Range of support provided, including consideration of support facilities personnel and flexibility in meeting requirements of the Center. CONTRACT DOCUMENTS: The attached Proposal Package contains the Statement of Work, Conditions of the Contract and Specifications describing the extent and requirements of the work. PERIOD OF PERFORMANCE: BASE YEARS - 01 August 2003 through 31 July 2004 OPTION YEAR 1 - 01 August 2004 through 31 July 2005 OPTION YEAR 2 - 01 August 2005 through 31 July 2006 OPTION YEAR 3 - 01 August 2006 through 31 July 2007 AWARD DATE: ON OR ABOUT - 29July 2003 The owner reserves the right to reject any and all proposals. INSTRUCTION TO OFFERER PROPOSAL Make proposal in accordance with the following instructions: 1. Submit offer on attached Proposal Form. Fill out all blanks, make no additions, erasures or alterations to the Proposal Form. An authorized official of the company making the offer must sign proposal Form. 2. Each bidder shall carefully examine the contract documents and shall review the existing Standing Orders, Job Corps Federal Regulations, Oral Health and Wellness program Technical Assistance Guide and the Job Corps Policy and Requirements Handbook (PRH), prior to submitting a proposal, to familiarize himself with all conditions under which the services are to be performed. 3. Deliver proposal to Jeremy Cotton, Purchasing Agent, Administration Building, Alaska Job Corps Center, 800 E. Lynn Martin Drive, Palmer, AK 99645 at or before the date/time stated in the Request For Proposal in a sealed opaque envelope marked "PROPOSAL FOR DENTAL SERVICES" and name of the bidder. Proposals delivered via Fed Ex, Express Mail, UPS, etc. are acceptable. Proposals received after the time stated in the Request For Proposal will be returned unopened. INTERPRETATION OF CONTRACT DOCUMENTS Should a bidder find discrepancies in, or omissions from, the proposal package, or should he be in doubt as to their meaning, he shall contact the Purchasing Agent, Jeremy Cotton, (907)746-8803, for an interpretation thereof prior to five working days of the time of proposal due date. The interpretation of the documents will be made by written Addendum; a copy of which will be issued to each bidder having received a Request for Proposal. ADDENDA OR BULLETINS Any Addenda issued prior to the proposal due date become part of the contract documents; shall be covered in the proposal; and become a part of the contract. AWARDS OR REJECTION OF BIDS The contract, if awarded, will be to the most qualified lowest responsive responsible bidder. The owner, however, shall be the sole judge as to who is responsive and responsible and reserves the right to reject any or all bids and to waive any irregularity or technicality in proposals received. BIDDER QUALIFICATIONS All personnel providing services must have a dental degree and be licensed currently by the State of Alaska Board of Dentistry. PATIENT POPULATION Services are to be provided to all students attending the Center. The Center is designed to have 250 students with ages ranging from approximately 16 to 26 years old. It is expected that the majority of students will be in the 16 to 21 year range with an approximately even numbers of male and females. DOCUMENTS TO BE FURNISHED WITH ALL PROPOSALS ? Lynn Martin Palmer, Alaska 99645 The undersigned agrees to provide students of the Alaska Job Corps Center, referred to henceforth as the Center, with the dental services as stated in the Statement of Work. Dental services will be provided for an estimated average of (16) hours per week, Dental Assistant (20) hours per week, and Dental Hygienist (4) hours per week for 50 weeks per contract year for all three positions. The services will be provided at the facilities located at the Center. GENERAL OUTLINE Services to be provided will include, but not limited to, those services specified in the Job Corps Federal Regulations (Title 20 Code of Federal Regulations, Part 638.510) and in the Job Corps Policy and Requirements Handbook (PRH), Chapter 6. Services include: 1. Provide direct oral health services to all Alaska Job Corps Center Students. 2. Assist in the development, implementation, and monitoring of the total Center Oral Health and Wellness Program. The extent of the work is further described in Section III, Extended Statement of Work SECTION III EXTENDED STATEMENT OF WORK A. The services provided shall include but not be limited to those Dental Services enumerated in the Job Corps Policy and Requirements Handbook (PRH), Chapter 5 .These services include: 1. The provisions of direct dental services to Alaska Job Corps Center students as follows: (a) A mandated oral examination and the development of an oral health and wellness plan, is to be accomplished forty-five to seventy-five (45-75) days after enrollee?s arrival on Center. (b) Written Diagnosis and treatment plan for each enrollee as part of an oral health and wellness plan and obtaining agreement for voluntary care before proceeding with treatment. On the basis of the diagnosis and with program constraints priorities for the receipt of comprehensive dental care will be established using Job Corps priority Classification System. (c) Basic oral care, as defined by the PRH and Technical Assistance Guide 6-A, and within budget constraints at the facility. (d) Development of an appropriate referral system to ensure the receipt of special care. (e) Twenty-four (24) hour emergency service. (f) Participating in the coordination and integration of the oral Health program with the wellness components, center-community activities. (g) Collection of data on productivity, oral health status, and oral health outcomes the first month of each quarter as described in the Oral Health and Wellness Program TAG. 2. Assist in the development, implementation and monitoring of the total Center Oral Health and Wellness Program. (a) Advise the Center Director and Health Services staff on all dental related matters. This shall include staffing requirements, the advisability of separations for dental reasons, the cost of services and any other areas deemed appropriate. (b) Provision of assistance to the Center Director and/or Center Physician in developing the coordination and integration of the dental health program with the other health components and the other Center Activity. (c) Ensure the maintenance, safeguarding and confidentiality of all required dental records, in compliance with federal, state and Job Corps guidelines. (d) Involvement in the development and implementation of the dental health education component. (e) Identification and utilization of the most appropriate and least expensive sources for obtaining necessary dental supplies and medications using AKJCC ordering forms, format and purchasing policies. (f) Review and sign standing orders annually for dental care in accordance with the Technical Assistance Guide on Standing Orders (6-Q). (g) Keeping current information on OSHA requirements for dental facilities and advising Health Services Manager on areas, which need modification to keep in compliance with these regulations. (h) Develop and comply with Job Corps infections control policies and procedures. (i) Provide support to the students in aquiring the oral health-related skills, knowledge, and attitudes that will make them employable. (j) Ensure that regular preventive maintenance is provided to the dental equipment. (k) Cooperate with corporate and Regional Office center assessments. (l) Maintain the accuracy and confidentiality of all required oral health and wellness records when they are in the subcontractor?s presence; comply with center?s HIPPA procedures for the protection of student?s health information. 4. Providing a dental assistant to directly assist the dentist in performing services, plus provide approximately, twenty (20) hours per week: (a) Schedule Students (b) Perform a cursory oral examination on new students (c) Assist in ordering, inventory maintenance and managing supplies. (d) Assist in completing and maintaining dental records. (e) Compiling statistical information and submitting regular monthly/special reports. (f) Conduct preventative lectures as requested by the Health Services Manager. 5. Providing a dental hygienist approximately four (4) hours per week to conduct preventative dental procedures to include, but not limited to: (a) Teeth cleaning (b) Tooth brush and dental floss instructions (c) Topical fluoride treatment (d) Sealant procedures
